基于聚焦解决模式的体位管理在急性心力衰竭机械通气中的应用

摘　　要：目的:观察基于聚焦解决模式的体位管理在急性心力衰竭机械通气中的应用价值。方法:选取2021年1月—2023年2月南阳市第一人民医院收治的110例急性心力衰竭患者作为研究对象,采用计算机随机分组法将其分为常规组和试验组,每组各55例。所有患者均实施机械通气治疗,常规组实施常规管理,试验组实施基于聚焦解决模式的体位管理,比较两组患者的肺部感染情况、肺动脉血气指标及肺顺应性(TC)、治疗情况、并发症发生情况。结果:管理后,试验组C-反应蛋白(CRP)、降钙素原(PCT)、白细胞(WBC)、动脉二氧化碳分压(PCO_(2))、预后营养指数(PNI)、主观整体营养状况评分(SGA)、机械通气时间、住院时间均低于常规组,动脉氧分压(PO2)、TC均高于常规组,差异均有统计学意义(t=6.815、8.898、5.052、2.520、3.358、3.668、5.488、5.869、3.839、2.741,P<0.05);试验组的并发症发生率低于常规组,差异有统计学意义(χ^(2)=6.201,P<0.05)。结论:基于聚焦解决模式下的体位管理能增强急性心力衰竭患者的机械通气治疗效果。有利于缓解肺部感染情况、促进肺动脉血气及TC恢复,并改善患者营养状况,降低并发症发生风险。Objective:To observe the application value of position management based on focused solution mode in mechanical ventilation for acute heart failure.Methods:110 patients with acute heart failure were selected as the study subjects and randomly divided into two groups,with 55 cases each.All patients underwent mechanical ventilation treatment,with the conventional group undergoing routine management,and the experimental group undergoing position management based on a focused solution model.The lung infection status,pulmonary arterial blood gas indicators and lung compliance(TC),treatment status,and incidence of complications were compared between the two groups of patients.Results:After management,CRP,PCT,WBC,PCO_(2),PNI,SGA,mechanical ventilation time and hospital stay in experimental group were lower than those in conventional group,while PO2 and TC were higher than those in conventional group.The differences were statistically significant(t=6.815,8.898,5.052,2.520,3.358,3.668,5.488,5.869,3.839,2.741;P<0.05).The complication rate of experimental group was lower than that of conventional group,and the difference was statistically significant(χ^(2)=6.201,P<0.05).Conclusion:Positional management based on a focused solution model can enhance the effectiveness of mechanical ventilation treatment in patients with acute heart failure.It is beneficial for alleviating pulmonary infection,promoting the recovery of pulmonary arterial blood gas and TC,improving patient nutritional status,and reducing the risk of complications.
