氧化锆全瓷冠联合玻璃纤维桩修复牙体缺损的疗效及其预后的影响因素研究

摘　　要：目的:探讨氧化锆全瓷冠联合玻璃纤维桩修复牙体缺损的疗效及其预后的影响因素。方法:选取2016年1月—2023年2月厦门市第五医院收治的600例牙体缺损患者进行回顾性分析。其中,将实施金属桩钴铬烤瓷冠修复的100例患者纳入对照组,将实施氧化锆全瓷冠联合玻璃纤维桩修复的500例患者纳入观察组。比较两组患者修复成功率与满意度,比较两组患者修复前后咀嚼功能与美学评分。随后将观察组患者中25例氧化锆全瓷冠联合玻璃纤维桩修复失败的患者纳入失败组,将475例氧化锆全瓷冠联合玻璃纤维桩修复成功的患者纳入成功组。经logistic回归分析牙列缺损修复失败的影响因素。结果:观察组患者手术成功率高于对照组,观察组患者总满意度高于对照组,差异均有统计学意义(χ^(2)=5.050、6.730,P<0.05)。修复后3个月、6个月,观察组患者咀嚼功能、美学评分高于对照组,差异均有统计学意义(t=5.572、9.498、8.691、7.649,P<0.05)。成功组和失败组患者口腔卫生习惯、黏结面内牙釉质百分比、菌斑控制不佳情况比较,差异均有统计学意义(χ^(2)=7.751、59.066、75.219,P<0.05)。经logistic回归分析结果显示,口腔卫生习惯不良、黏结面内牙釉质百分比为≥50%、菌斑控制不佳是牙列缺损修复失败的影响因素。结论:氧化锆全瓷冠联合玻璃纤维桩修复牙体缺损的疗效显著,能够改善患者的咀嚼功能与美学评价。口腔卫生习惯不良、黏结面内牙釉质百分比为≥50%、菌斑控制不佳是牙列缺损修复失败的影响因素。Objective:To investigate the effect and prognostic factors of zirconia all-ceramic crown combined with glass fiber post in the repair of tooth defects.Methods:600 patients with dental defects admitted to the hospital from January 2016 to February 2023 were selected for retrospective analysis.Among them,100 patients who implemented metal pile cobalt-chromium porcelain crown restoration were included in the control group,and 500 patients who implemented zirconia all-ceramic crown combined with glass fiber pile restoration were included in the observation group.The success rate and satisfaction of the two groups were compared,and the masticatory function and aesthetic score of the two groups were compared before and after the repair.Then,25 patients who failed to repair tooth defects were divided into failure group,and the other 475 patients were divided into success group.Logistic regression analysis was used to analyze the multiple factors affecting the prognosis of tooth defects.Results:The surgical success rate of the observation group was significantly higher than that of the control group,and the total satisfaction rate of the observation group was significantly higher than that of the control group(χ^(2)=5.050,6.730;P<0.05).The masticatory function and aesthetic score of the two groups were increased 3 and 6 months after repair,and the observation group was higher than the control group(t=5.572,9.498,8.691,7.649;P<0.05).There were significant differences in oral hygiene habits,repair methods,percentage of enamel in the bonding surface and poor plaque control between the successful group and the failure group(χ^(2)=7.751,59.066,75.219;P<0.05).Logistic regression analysis showed that poor oral hygiene habits,percentage of enamel in adhesive surface≥50%and poor plaque control were the factors affecting the failure of dental defect repair.Conclusion:Zirconia porcelain crown combined with glass fiber post can improve the masticatory function and aesthetic evaluation of patients.Poor oral hygiene habits,the per
