左西孟旦治疗应激性心肌病患者临床疗效的汇总分析

摘　　要：目的 汇总分析评价左西孟旦治疗应激性心肌病患者的临床疗效。方法 以左西孟旦和应激性心肌病为检索词,分别检索Pubmed、Web of Science、Cochrane Library、Springer、中国知网、万方数据库、维普数据库。检索左西孟旦治疗应激性心肌病患者的临床试验,结局指标为死亡率、住院时间、左室射血分数等。应用RevMan 5.4对纳入的研究进行数据分析。结果 汇总分析共纳入5篇文献,共有298例患者,其中对照组150例,左西孟旦组148例。结果显示,与对照组比较,左西孟旦组应激性心肌病患者死亡率降低[OR=0.30,95%CI(0.09,1.00),Z=1.96,P=0.05],住院时间明显缩短[MD=-4.91,95%CI(-5.87,-3.94),Z=9.93,P<0.05],左室射血分数增高[MD=3.81,95%CI(2.20,5.41),Z=4.65,P<0.05]。结论 左西孟旦能降低应激性心肌病患者的死亡率,缩短住院时间,增加左室射血分数,促进心脏功能的恢复。Objective To evaluate the clinical efficacy of levosimendan in the treatment of patients with stress cardio⁃myopathy by meta-analysis.Methods With levosimendan and stress cardiomyopathy as search terms,Pubmed,Web of Science,Cochrane Library,Springer,CNKI,Wanfang database and VIP database were searched respectively.The clinical trials of levosimendan in the treatment of patients with stress cardiomyopathy were retrieved.The outcome in⁃dicators were mortality,hospitalization time,left ventricular ejection fraction,etc.RevMan 5.4 was used to analyze the data of the included studies.Results A total of 5 articles were included in the meta-analysis,with a total of 298 pa⁃tients,including 150 in the control group and 148 in the levosimendan group.The results showed that compared with the control group,the mortality rate of patients with stress cardiomyopathy in the levosimendan group was lower[OR=0.30,95%CI(0.09,1.00),Z=1.96,P=0.05],the hospitalization time was significantly shorter[MD=-4.91,95%CI(-5.87,-3.94),Z=9.93,P<0.05],and the left ventricular ejection fraction was increased[MD=3.81,95%CI(2.20,5.41),Z=4.65,P<0.05].Conclusion Levosimendan can reduce the mortality of patients with stress cardiomyopathy,shorten the length of hospital stay,increase left ventricular ejection fraction,and promote the recovery of cardiac func⁃tion.
