环泊酚与丙泊酚在胸科手术麻醉诱导中的应用效果

摘  要:目的 探讨环泊酚与丙泊酚在胸科手术麻醉诱导中的应用及对血流动力学状态、认知功能的影响。方法回顾性选取2023年5—12月徐州市肿瘤医院行胸科手术的90例患者的临床资料,依据麻醉诱导用药的不同分为丙泊酚组(n=45)和环泊酚组(n=45)。两组患者均联合顺阿曲库铵、舒芬太尼、咪达唑仑进行麻醉诱导,同时丙泊酚组给予静脉推注2 mg/kg丙泊酚乳状注射液,环泊酚组给予静脉推注0.4 mg/kg环泊酚注射液进行麻醉诱导。比较两组血流动力学指标,认知功能,术后恢复效率及不良反应。结果 相较于丙泊酚组,麻醉诱导后(T_(1))、气管插管后5 min(T_(3)),环泊酚组心率、血压水平均较高,气管插管即刻(T_(2)),环泊酚组心率血压水平均较低,差异均有统计学意义(P均<0.05)。气管插管后5 min(T_(3))时,环泊酚组血压、心率高于丙泊酚组,差异均有统计学意义(P均<0.05)。环泊酚组术后2 h认知功能评分高于丙泊酚组,差异有统计学意义(P<0.05)。环泊酚组自主呼吸恢复时间、睁眼时间、麻醉恢复室停留时间、拔管时间、首次下床活动时间均短于丙泊酚组,差异均有统计学意义(P均<0.05)。环泊酚组总不良反应总发生率为4.44%(2/45),低于丙泊酚组的17.78%(8/45),差异有统计学意义(χ^(2)=4.050,P<0.05)。结论 环泊酚与丙泊酚均能用于胸科手术麻醉诱导中,其中环泊酚对于血流动力学状态影响较小,术后患者恢复较早,不良反应发生率较低,安全性较高,值得临床借鉴。Objective To investigate the application of cyclopropofol and propofol in anesthesia induction in thoracic surgery and the effects on hemodynamic status and cognitive function.Methods The clinical data of ninety patients who underwent thoracic surgery in Xuzhou Cancer Hospital from May to December 2023 were retrospectively selected.According to the different anesthesia induction drugs,they were divided into propofol group(n=45)and cyclopropofol group(n=45).Both groups were combined with cisatracurium,sufentanil and midazolam for anesthesia induction.At the same time,the propofol group was given intravenous injection of 2 mg/kg propofol emulsion injection,and the cy⁃clopropofol group was given intravenous injection of 0.4 mg/kg cyclopropofol injection for anesthesia induction.The hemodynamic indexes,cognitive function,postoperative recovery efficiency and adverse reactions were compared be⁃tween the two groups.Results Compared with the propofol group,the heart rate and blood pressure levels of the propo⁃fol group were higher after anesthesia induction(T_(1))and 5 min after tracheal intubation(T_(3)),and the heart rate and blood pressure levels of the Cyclopophenol group were lower immediately after tracheal intubation(T_(2)),and the differ⁃ences were statistically significant(all P<0.05).The cognitive function score of the cyclopropofol group was higher than that of the propofol group at 2 h after operation,and the difference was statistically significant(P<0.05).The re⁃covery time of spontaneous breathing,the time of opening eyes,the time of staying in PACU,the time of extubation and the time of first ambulation in the cyclopropofol group were shorter than those in the propofol group,and the differ⁃ences were statistically significant(all P<0.05).The total incidence of adverse reactions in the cyclopropofol group was 4.44%(2/45),which was lower than 17.78%(8/45)in the propofol group,and the difference was statistically sig⁃nificant(χ^(2)=4.050,P<0.05).Conclusion Both cyclopropofol and propofol can
