血液透析联合血液灌流在老年慢性肾脏病患者中的应用效果

摘　　要：目的 分析血液透析联合血液灌流的应用价值。方法 随机选取2020年1月—2023年12月桂东县中医医院收治的100例老年慢性肾脏病患者为研究对象。根据治疗方案不同分为两组,各50例。对照组仅采取血液透析治疗,观察组采取血液透析联合血液灌流。比较两组疗效、并发症发生情况及矿物质、骨代谢状况。结果 观察组总有效率为98.0%(49/50)比对照组的80.0%(40/50)更高,差异有统计学意义(χ^(2)=8.274,P<0.05)。观察组患者皮肤瘙痒、低血压及失衡综合征的发生率低于对照组,差异有统计学意义(P<0.05)。治疗后观察组血钙(2.78±0.32)mmol/L、骨形态发生蛋白3(0.59±0.14)ng/mL高于对照组的(2.35±0.25)mmol/L、(0.48±0.12)ng/mL,血磷(2.02±0.32)mmol/L、甲状旁腺激素(209.59±20.14)ng/mL低于对照组的(2.32±0.45)mmol/L、(256.48±24.12)ng/mL,,差异均有统计学意义(t=5.432,5.468,5.326,5.326;P均<0.05)。结论 血液透析联合血液灌流的疗效与安全性高,能够有效改善患者的矿物质、骨代谢状况。Objective To analyze the application value of hemodialysis combined with hemoperfusion.Methods A to⁃tal of 100 elderly patients with chronic kidney disease admitted to Guidong County Hospital of Traditional Chinese Medicine from January 2020 to December 2023 were randomly selected as the research objects.According to differ⁃ent treatment regimens,they were divided into two groups,50 cases in each group.The control group was treated with hemodialysis,and the observation group was treated with hemodialysis combined with hemoperfusion.The curative effect,complications,mineral and bone metabolism were compared between the two groups.Results The total effec⁃tive rate of the observation group was 98.0%(49/50),which was higher than 80.0%(40/50)of the control group,and the difference was statistically significant(χ^(2)=8.274,P<0.05).The incidence of skin itching,hypotension and imbalance syndrome in the observation group was lower than that in the control group,and the difference was statisti⁃cally significant(P<0.05).After treatment,the blood calcium(2.78±0.32)mmol/L and bone morphogenetic protein 3(0.59±0.14)ng/mL in the observation group were higher than those in the control group(2.35±0.25)mmol/L and(0.48±0.12)ng/mL,blood phosphorus(2.02±0.32)mmol/L and parathyroid hormone(209.59±20.14)ng/mL were lower than(2.32±0.45)mmol/L and(256.48±24.12)ng/mL in the control group,the differences were statistically significant(t=5.432,5.468,5.326,5.326;all P<0.05).Conclusion Hemodialysis combined with hemoperfusion has high efficacy and safety,and can effectively improve the mineral and bone metabolism of patients.
