氯吡格雷联合静脉溶栓治疗冠心病并发急性心肌梗死患者的临床疗效及对心功能的影响

摘　　要：目的 探讨冠状动脉粥样硬化性心脏病(简称冠心病)合并急性心肌梗死患者采用氯吡格雷+静脉溶栓治疗的临床效果。方法 回顾性选取2021年6月至2023年12月于梁山县人民医院收治的60例冠心病合并急性心肌梗死患者的治疗资料,依据用药方案不同分为两组,每组30例。参照组使用氯吡格雷,研究组在参照组基础上给予静脉溶栓治疗,比较两组患者的治疗效果、不良反应、心功能指标。结果 研究组的总有效率为96.67%(29/30),高于参照组的73.33%(22/30),差异有统计学意义(χ^(2)=4.705,P<0.05)。研究组的心功能指标优于参照组,差异均有统计学意义(P均<0.05)。两组的不良反应总发生率比较,差异无统计学意义(P>0.05)。结论 在冠心病合并急性心肌梗死患者的治疗中,联合使用氯吡格雷+静脉溶栓可提升疗效,有效改善心功能,且安全性良好。Objective To investigate the clinical effect of clopidogrel and intravenous thrombolysis in patients with coronary atherosclerotic heart disease(CHD)complicated with acute myocardial infarction.Methods The treatment data of sixty patients with coronary heart disease complicated with acute myocardial infarction admitted to Liangshan County People's Hospital from June 2021 to December 2023 were retrospectively selected and divided into two groups with thirty cases in each group according to different medication regimen.The reference group received clopidogrel,and the study group received intravenous thrombolytic therapy based on the reference group.The therapeutic effect,adverse reactions and cardiac function indexes of the two groups were compared.Results The total effective rate of the study group was 96.67%(29/30),higher than 73.33%(22/30)of the reference group,and the difference was statisti⁃cally significant(χ^(2)=4.705,P<0.05).The cardiac function index of the study group were better than thoset of the refer⁃ence group,and the differences were statistically significant(all P<0.05).There was no significant difference in the to⁃tal incidence of adverse reactions between the two groups(P>0.05).Conclusion In the treatment of patients with coro⁃nary heart disease complicated with acute myocardial infarction,the combination of clopidogrel and intravenous throm⁃bolysis can improve the efficacy,effectively improve cardiac function,and have good safety.
