重型颅脑损伤患者行扩大翼点入路控制性减压手术的治疗效果及其对神经功能的影响

摘  要:目的 探究重型颅脑损伤患者行扩大翼点入路控制性减压手术治疗效果。方法 方便选取2020年1月—2023年12月南京市高淳中医院神经外科收治的50例重型颅脑损伤患者为研究对象,根据不同术式分组,A组进行扩大翼点入路控制性减压手术治疗,B组进行标准大骨瓣减压术+常规快速减压治疗,每组25例。比较两组颅内压水平、神经功能、术后并发症、预后恢复情况。结果 术后7 d,A组颅内压为(14.45±1.37)mmHg,低于B组的(20.15±2.09)mmHg,差异有统计学意义(t=11.405,P<0.05)。A组神经功能、预后恢复情况优于B组,差异均有统计学意义(P均<0.05)。两组并发症发生率比较,差异均无统计学意义(P均>0.05)。结论 扩大翼点入路控制性减压手术疗效明显,可降低患者颅内压水平,改善神经功能,促进预后恢复。Objective To investigate the efficacy of expanded wing point approach controlled decompression surgery in patients with severe craniocerebral injuries.Methods Fifty patients with heavy craniocerebral injuries admitted to the Neurosurgery Department of Nanjing Gaochun Hospital of Traditional Chinese Medicine from January 2020 to De⁃cember 2023 were conveniently selected as the study subjects,and were grouped according to different surgical styles.Group A underwent controlled decompression surgical treatment with enlarged wing point approach and group B un⁃derwent standard large bone flap decompression+conventional rapid decompression treatment,twenty-five cases in each group.The levels of intracranial pressure,neurologic function,postoperative complications,and prognostic recov⁃ery were compared between the two groups.Results At 7 d after operation,the intracranial pressure of group A was(14.45±1.37)mmHg,lower than(20.15±2.09)mmHg in group B,and the difference was statistically significant(t=11.405,P<0.05).The neurological function and prognostic recovery of group A were better than those of group B,and the differences were statistically significant(both P<0.05).There were no statistically significant differences in the complication rates between the two groups(all P>0.05).Conclusion The efficacy of controlled decompression surgery with expanded pterygoid point approach is obvious,which can reduce patients'intracranial pressure level,improve neurological function,and promote prognostic recovery.
