摘　　要：目的:探讨坤宁颗粒联合口服避孕药对异常子宫出血患者子宫动脉血流及细胞因子水平的影响。方法:选择2021年3月至2024年3月收治的80例功能失调性子宫出血患者为研究对象并行前瞻性研究,受试者根据随机数字表法予以分组,各40例,对照组(口服避孕药)与研究组(坤宁颗粒+口服避孕药),对比两组患者治疗效果与各细胞因子水平变化、子宫动脉血流、子宫出血与月经失血图(PBAC)评分及子宫内膜厚度变化以及用药安全性。结果:研究组总疗效比对照组更高(P<0.05);研究组相关治疗时间指标均较对照组短(P<0.05);组间各血清细胞因子水平相比,治疗后研究组PGE2与Ang-Ⅱ水平较对照组低,β-EP水平较对照组高(P<0.05);组间PI、RI及S/D相比,治疗后研究组较对照组低(P<0.05);组间子宫出血与PBAC评分及子宫内膜厚度相比,治疗后研究组子宫出血评分与子宫内膜厚度较对照组低,PBAC评分较对照组高(P<0.05);组间用药安全性相比,数据评比结果显示无差异性(P>0.05)。结论:在异常子宫出血治疗中联合应用坤宁颗粒与口服避孕药可有效调节各细胞因子水平,改善子宫动脉血流及子宫内膜厚度,疗效显著且安全,对促进患者病情康复具有积极意义。Objective:To investigate the effects of Kunning granules combined with oral contraceptives on uterine artery blood flow and cytokine level in patients with abnormal uterine bleeding.Methods:80 patients with abnormal uterine bleeding admitted from March 2021 to March 2024 were selected as research objects for a parallel prospective study,Subjects were divided into 40 groups according to random number table method,Control group(oral contraceptives)and study group(Kunning granules+oral contraceptives),The therapeutic effect,changes in levels of sex hormones and cytokines,uterine artery blood flow,uterine bleeding and PBAC score,changes in endometrial thickness and drug safety were compared between the two groups.Results:Compared with other groups,the treatment effect was higher in the study group(P<0.05).The relevant treatment time indexes in the study group were shorter than those in the control group,and the difference was statistically significant(P<0.05).The levels of serum cytokines in the study group were lower than those in the control group,and the levels ofβ-EP were higher than those in the control group after treatment(P<0.05).PI,RI and S/D were lower in the study group than in the control group after treatment(P<0.05).The results showed that after treatment,the uterine bleeding score and endometrial thickness of the study group were lower than that of the control group,and the PBAC score was higher than that of the control group(P<0.05).There was no difference in drug safety between groups(P>0.05).Conclusion:The combined application of Kunning granules and oral contraceptives in the treatment of abnormal uterine bleeding can effectively regulate the levels of various cytokines,improve the uterine artery blood flow and endometrial thickness,with significant efficacy and safety,and has positive significance in promoting the rehabilitation of patients.
