长期应用炔雌醇环丙孕酮片与去氧孕烯炔雌醇片治疗多囊卵巢综合征不孕症患者的疗效

摘　　要：目的探讨长期应用炔雌醇环丙孕酮片与去氧孕烯炔雌醇片治疗多囊卵巢综合征(PCOS)不孕症患者的疗效。方法选取2021年1月至2023年12月中国人民解放军西部战区总医院收治的60例PCOS不孕症患者作为研究对象,按照随机信封法分为甲组和乙组,每组30例。甲组采用炔雌醇环丙孕酮片治疗,乙组采用去氧孕烯炔雌醇片治疗。比较两组临床疗效、临床指征、性激素水平、代谢指标水平、不良反应发生率和妊娠率。结果甲组治疗总有效率高于乙组(P<0.05)。治疗后,两组子宫内膜厚度、成熟卵泡个数、最大卵泡直径均升高,且甲组高于乙组(P<0.05)。治疗后,两组卵泡刺激素(FSH)、雌二醇(E 2)水平升高且甲组高于乙组,两组黄体生成素(LH)水平降低且甲组低于乙组(P<0.05)。治疗后,两组空腹血糖(FPG)、空腹胰岛素(FINS)、胰岛素抵抗指数(HOMA-IR)水平均降低且甲组低于乙组,两组甘油三酯(TG)、总胆固醇(TC)水平升高(P<0.05)。两组不良反应总发生率比较,差异无统计学意义(P>0.05)。甲组妊娠率高于乙组(P<0.05)。结论与去氧孕烯炔雌醇片比较,长期应用炔雌醇环丙孕酮片治疗PCOS不孕症疗效更好,能够更好地改善患者子宫和卵巢功能,调节性激素和糖代谢水平,提高妊娠率,且不会增加不良反应。Objective To explore the efficacy of long-term application of ethinylestradiol-cyproterone acetate tablets and desogestrel-ethinylestradiol tablets in treating infertility patients with polycystic ovary syndrome(PCOS).Methods A total of sixty infertility patients with PCOS admitted to General Hospital of the Western Theater Command of the Chinese People′s Liberation Army from January 2021 to December 2023 were selected as research subjects,and were divided into group A and group B by adopting random envelop method,with 30 cases in each group.Group A was treated with ethinylestradiol-cyproterone acetate tablets,while group B was treated with desogestrel-ethinylestradiol tablets.The clinical efficacy,clinical indications,sex hormone levels,metabolic indicator levels,incidence of adverse reactions and pregnancy rate were compared between the two groups.Results The total effective rate of treatment in group A was higher than that in group B(P<0.05).After treatment,the endometrial thickness,mature follicle count and maximum follicle diameter in both groups were all increased,and the levels in group A was higher than those in group B(P<0.05).After treatment,the levels of follicle stimulating hormone(FSH)and estradiol(E 2)in the two groups were increased,and the levels in group A was higher than those in group B;the levels of luteinizing hormone(LH)in the two groups were decreased,and the level in group A was lower than that in group B(P<0.05).After treatment,the levels of fasting plasma glucose(FPG),fasting insulin(FINS)and insulin resistance index(HOMA-IR)were all decreased in the two groups,and the levels in group A was lower than those in group B;the levels of triglyceride(TG)and total cholesterol(TC)in both groups were increased(P<0.05).There was no statistical significance in the total incidence of adverse reactions between the two groups(P>0.05).The pregnancy rate in group A was higher than that in group B(P<0.05).Conclusions Compared with desogestrel-ethinylestradiol tablets,long-term application of ethinylest
