自拟疏肝化痰方联合针刺对痰湿型多囊卵巢综合征患者中医证候评分及血清性激素水平的影响

摘　　要：目的分析自拟疏肝化痰方联合针刺对痰湿型多囊卵巢综合征(PCOS)患者中医证候评分及血清性激素水平的影响。方法选取2021年1月至2023年6月石家庄市中医院收治的90例痰湿型PCOS患者作为研究对象。采用随机数字表法分为中药组(n=45,给予自拟疏肝化痰方联合调经助孕针刺治疗)和激素组(n=45,给予屈螺酮炔雌醇片治疗)。比较两组的中医证候评分、血清性激素水平、卵泡数量、卵巢体积、排卵率及妊娠率。结果治疗后,两组月经异常、形体肥胖、带下、身体乏力倦怠、头晕犯恶、胸闷心悸等中医证候评分均下降,且中药组低于激素组(P<0.05)。与治疗前比较,治疗后两组雌二醇(E_(2))水平上升,黄体生成素(LH)、催乳素(PRL)、睾酮(T)水平下降(P<0.05);且中药组E_(2)水平高于激素组,LH、T水平低于激素组(P<0.05)。治疗后,两组卵泡数量、卵巢体积均减少,且中药组少于激素组(P<0.05)。治疗后,中药组妊娠率显著高于激素组(P<0.05)。两组排卵率比较,差异无统计学意义(P>0.05)。结论自拟疏肝化痰方联合针刺治疗痰湿型PCOS患者,可降低中医证候评分,调节血清性激素水平,提高妊娠率。Objective To analyze the effects of self-made Shugan Huatan Recipe combined with acupuncture and moxibustion on traditional Chinese medicine(TCM)syndrome score and serum sex hormone levels in patients with phlegm dampness type polycystic ovary syndrome(PCOS).Methods A total of 90 patients with phlegm dampness type PCOS admitted to the Traditional Chinese Medicine Hospital of Shijiazhuang from January 2021 to June 2023 were selected as the research objects.The patients were divided into a TCM group(n=45,treated with self-made Shugan Huatan Recipe combined with regulating menstruation and assisting pregnancy acupuncture)and a hormone group(n=45,treated with drosterone ethinylestradiol tablets).The TCM syndrome scores,hormone levels,number of follicles and ovarian volume,ovulation rate and pregnancy rate of the two groups were compared.Results After treatment,the TCM syndrome score of menstrual abnormalities,body obesity,vaginal discharge,fatigue,dizziness and nausea,chest tightness and palpitations,etc.in both groups decreased,and those in the TCM group were lower than those in the hormone group(P<0.05).Compared with before treatment,the levels of estradiol(E_(2))increased,the levels of luteinizing hormone(LH),prolactin(PRL),and testosterone(T)decreased after treatment in both groups(P<0.05),and the level of E_(2) in the TCM group were higher than that in the hormone group,while the levels of LH and T were lower than those in the hormone group(P<0.05).After treatment,the number of follicles and ovarian volume in both groups decreased,and those in the TCM group were lower than those in the hormone group(P<0.05).After treatment,the pregnancy rate in TCM group was significantly higher than that in the hormone group(P<0.05).There was no significant difference in ovulation rate between the two groups(P>0.05).Conclusions Self-made Shugan Huatan Recipe combined with acupuncture and moxibustion in the treatment of phlegm dampness type PCOS patients can reduce TCM syndrome score,regulate serum sex hormone levels,and improve
