温经通络方联合盐酸左氧氟沙星胶囊治疗盆腔炎后遗盆腔痛对疼痛程度及Th1/Th2平衡的调节作用

摘　　要：目的探讨温经通络方联合盐酸左氧氟沙星胶囊治疗盆腔炎后遗盆腔痛对疼痛程度及辅助性T细胞(Th)1/Th2平衡的调节作用。方法选取2022年8月至2024年8月四川宝石花医院收治的160例盆腔炎后遗盆腔痛患者作为研究对象,根据随机数字表法分为观察组和对照组,每组80例。对照组采用盐酸左氧氟沙星胶囊联合布洛芬缓释胶囊治疗,观察组在对照组基础上采用温经通络方治疗。比较两组临床疗效、疼痛证候积分、Th1/Th2平衡情况、炎性因子水平、不良反应发生率。结果观察组临床总有效率高于对照组(P<0.05)。治疗后,两组子宫压痛、附件区压痛、宫骶韧带触痛评分及总分均降低,且观察组均低于对照组(P<0.05)。治疗后,两组Th1、Th1/Th2升高且观察组高于对照组,两组Th2降低且观察组低于对照组(P<0.05)。治疗后,两组肿瘤坏死因子-α(TNF-α)、γ干扰素(IFN-γ)、白介素-2(IL-2)水平均降低,且观察组均低于对照组(P<0.05)。两组不良反应总发生率比较,差异无统计学意义(P>0.05)。结论温经通络方联合盐酸左氧氟沙星胶囊治疗盆腔炎后遗盆腔痛疗效良好,可缓解患者疼痛程度,促进Th1/Th2平衡,且不会增加不良反应。Objective To explore the regulation effect of Wenjing Tongluo Prescription combined with levofloxacin hydrochloride capsule on pain severity and T helper cell(Th)1/Th2 balance in treating pelvic pain after pelvic inflammation.Methods A total of 160 patients with pelvic pain after pelvic inflammation admitted to Sichuan Gemflower Hospital between August 2022 and August 2024 were enrolled as research objects.According to random number table method,they were divided into control group and observation group,with 80 patients in each group.The control group was treated with levofloxacin hydrochloride capsules combined with ibuprofen sustained-release capsules,and the observation group was treated with Wenjing Tongluo Prescription on the basis of control group.The clinical curative effect,scores of pain syndromes,Th1/Th2 balance condition,inflammatory factors level and incidence of adverse reactions were compared between the two groups.Results The total clinical effective rate in the observation group was higher than that in the control group(P<0.05).After treatment,the scores of uterine tenderness,appendicular tenderness,uterosacral ligament tenderness and the total score were all decreased in both groups,and those in the observation group were lower than those in the control group(P<0.05).After treatment,Th1 and Th1/Th2 were increased in both groups,and those were higher in the observation group than in the control group.Th2 in both groups were decreased,and that was lower in the observation group than in the control group(P<0.05).After treatment,the levels of tumor necrosis factor-α(TNF-α),interferon γ(IFN-γ)and interleukin-2(IL-2)were all decreased in both groups,and those were lower in the observation group than in the control group(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusions The curative effect of Wenjing Tongluo Prescription combined with levofloxacin hydrochloride capsule is good in treating pelvic pain after pelvic inflam
