摘  要:目的分析膝关节骨性关节炎(KOA)患者外周血单个核细胞(PBMC)中核转录因子κB受体活化因子配体(RANKL)、前列腺素E2受体(PTGER2)、环氧化酶2(COX2)基因表达水平及意义。方法选取2018年1月至2023年12月宁夏医科大学总医院收治的100例KOA患者纳入KOA组,另外选取同时期30例健康志愿者纳入对照组。检测2组研究对象PBMC中RANKL、PTGER2、COX2基因表达水平。构建Logistic回归模型检验RANKL、PTGER2、COX2基因表达水平与KOA发生风险的关系。采用受试者工作特征曲线评估RANKL、PTGER2、COX2基因表达水平对KOA的诊断效能。结果KOA组RANKL、PTGER2、COX2基因表达水平明显高于对照组(P<0.05)。Logistic回归分析结果显示,RANKL、PTGER2、COX2基因表达水平升高与KOA的发生有关(P<0.05)。RANKL、PTGER2、COX2基因表达水平诊断KOA的曲线下面积(AUC)分别为0.828、0.876、0.800,3项联合诊断AUC为0.970(95%CI:0.924~0.992),显著高于单独指标(Z=8.986、11.959、7.285,均P<0.01)。3项联合诊断KOA的截断值为0.30,敏感度为91.00%,特异度为96.67%。结论KOA患者RANKL、PTGER2、COX2基因为高表达水平,与发生风险呈明显相关性,联合检测可提高诊断效能。Objective To analyze the expression levels and significance of receptor activator of nuclear factor-kappa B ligand(RANKL),prostaglandin E receptor 2(PTGER2)and cyclooxygenase-2(COX2)genes in peripheral blood mononuclear cells(PBMCs)from patients with knee osteoarthritis(KOA).Methods A total of 100 patients with KOA admitted to General Hospital of Ningxia Medical University from January 2018 to December 2023 were selected as the KOA group.Meanwhile,30 healthy volunteers were selected as the control group.The expression levels of RANKL,PTGER2,and COX2 genes in PBMCs of the two groups were detected.The Logistic regression model was used to analyze the relationship between the expression levels of RANKL,PTGER2 and COX2 genes and the risk of KOA.The diagnostic efficacy of the expression levels of RANKL,PTGER2 and COX2 genes for KOA was evaluated using the receiver operating characteristic curve.Results The expression levels of RANKL,PTGER2,and COX2 genes in the KOA group were significantly higher than those in the control group(P<0.05).Logistic regression analysis results showed that the expression levels of RANKL,PTGER2,and COX2 genes were related to the occurrence of KOA(P<0.05).The area under the curve(AUCs)of RANKL,PTGER2,and COX2 genes for diagnosing KOA were 0.828,0.876 and 0.800.The AUC of combined diagnosis with the three was 0.970(95%CI:0.924-0.992),which was significantly greater than that of single diagnosis(Z=8.986,11.959,7.285,P<0.01).The cutoff value,sensitivity and specificity of combined diagnosis of KOA were 0.30,91.00%and 96.67%.Conclusion The expression levels of RANKL,PTGER2,and COX2 genes in patients with KOA are high,and significantly correlated with the risk of KOA.Combined detection can enhance the diagnostic efficacy.
