加味逍遥颗粒联合帕罗西汀治疗围绝经期情绪障碍的疗效及对血清5-HT、DA、NE水平的影响

摘　　要：目的探讨加味逍遥颗粒联合帕罗西汀治疗围绝经期情绪障碍的临床疗效。方法选择2021年2月至2024年3月夹江县妇幼保健计划生育服务中心收治的101例围绝经期情绪障碍的患者为研究对象,按照随机数表法将患者分为对照组(50例)和研究组(51例),对照组患者接受帕罗西汀治疗,研究组患者接受帕罗西汀和加味逍遥颗粒的联合治疗,比较2组患者临床疗效、激素指标、单胺类神经递质指标、临床症状评分,记录2组治疗期间不良反应情况。结果研究组临床疗效高于对照组(P<0.05)。2组患者治疗8周后雌二醇升高,且研究组高于对照组(P<0.05);2组患者治疗8周后促黄体生成素、促卵泡激素降低,且研究组低于对照组(P<0.05)。2组患者治疗8周后5-羟色胺、多巴胺、去甲肾上腺素均升高,且研究组高于对照组(P<0.05)。2组患者治疗8周后汉密尔顿抑郁量表、汉密尔顿焦虑量表评分均下降,且研究组低于对照组(P<0.05)。2组患者不良反应发生率比较,差异无统计学意义(P>0.05)。结论加味逍遥颗粒联合帕罗西汀治疗围绝经期情绪障碍,可迅速改善患者临床症状,疗效明显,可有效改善激素指标及单胺类神经递质指标水平,且不增加不良反应发生率,临床应用价值较高。Objective To explore the clinical efficacy of Jiawei Xiaoyao Granules combined with paroxetine in the treatment of perimenopausal emotional disorders.Methods A total of 101 patients with perimenopausal emotional disorders were randomly divided into the control group(n=50)and the study group(n=51).The patients in the control group received paroxetine treatment,while those in the study group were given a combination of paroxetine and Jiawei Xiaoyao Granules.The clinical efficacy,hormone indexes,monoamine neurotransmitter indexes,and clinical symptom scores of the two groups were compared,and the adverse reactions during treatment in both groups were recorded.Results The clinical efficacy of the study group was higher than that of the control group(P<0.05).After 8 weeks of treatment,estradiol levels increased in both groups,and they were higher in the study group than in the control group(P<0.05);levels of luteinizing hormone and follicle-stimulating hormone decreased in both groups after 8 weeks of treatment,and the levels in the study group were lower than those in the control group(P<0.05).After 8 weeks of treatment,levels of 5-hydroxytryptamine dopamine,and norepinephrine increased in both groups,and the levels in the study group were higher than those in the control group(P<0.05).After 8 weeks of treatment,the scores of the Hamilton Depression Scale and the Hamilton Anxiety Scale decreased in both groups,and those in the study group were lower than those in the control group(P<0.05).There was no difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion The combination of Jiawei Xiaoyao Granules and paroxetine in the treatment of perimenopausal emotional disorders can rapidly improve the clinical symptoms of patients,demonstrate remarkable efficacy,effectively improve the levels of hormone indicators and monoamine neurotransmitter indicators,and avoid increase the incidence of adverse reactions,thus having a high clinical application value.
