不同剂量阿芬太尼复合右美托咪定在经皮椎骨成形术中的应用

摘　　要：目的探讨不同剂量阿芬太尼复合右美托咪定在经皮椎骨成形术中的临床应用效果。方法选取2022年3月至2024年8月在安徽省亳州市中医院行经皮椎骨成形术的90例胸腰椎压缩性骨折患者为研究对象。依据阿芬太尼剂量将患者随机分为L组(5μg/kg)、M组(7.5μg/kg)和H组(10μg/kg),每组30例。记录患者T0~T5时刻的心率(HR)、呼吸频率(RR)、血氧饱和度(SpO_(2))、平均动脉压(MAP)、脑电双频指数(BIS)、视觉模拟评分(VAS)及警觉/镇静观察评分(OAA/S),记录围术期不良事件,术后收集患者的麻醉满意度。结果M组与L组T0~T5时刻的HR、RR、SpO_(2)、MAP比较,差异均无统计学意义(P>0.05),但M组BIS、VAS评分及OAA/S评分均低于L组(P<0.05)。H组T0~T5时刻的HR、RR、SpO_(2)、MAP、BIS均低于L组(P<0.05),且VAS评分及OAA/S评分亦均低于L组(P<0.05)。H组T0~T5时刻的HR、RR、SpO_(2)、MAP、BIS均低于M组(P<0.05),而VAS评分及OAA/S评分2组间比较差异均无统计学意义(P>0.05)。3组患者满意度评分(M组>H组>L组)比较,差异有统计学意义(P<0.05)。结论7.5μg/kg阿芬太尼复合右美托咪定在经皮椎骨成形术中的应用效果最优。Objective To explore the clinical application effect of different doses of alfentanil combined with dexmedetomidine in percutaneous vertebroplasty.Methods A total of 90 patients with thoracolumbar compression fractures who underwent percutaneous vertebroplasty in Bozhou Hospital of Traditional Chinese Medicine from March 2022 to August 2024 were selected as the study subjects.According to the dose of alfentanil,the patients were randomly divided into the L group(5μg/kg),the M group(7.5μg/kg),and the H group(10μg/kg),with 30 cases in each group.The heart rate(HR),respiratory rate(RR),oxygen saturation(SpO_(2)),mean arterial pressure(MAP),bispectral index(BIS),visual analogue scale(VAS),and observer's assessment of alertness/sedation(OAA/S)of the patients at T0-T5 were recorded,perioperative adverse events were recorded,and the postoperative anesthesia satisfaction of the patients was collected.Results There was no statistically significant difference in HR,RR,SpO_(2),and MAP between the M group and the L group at T0-T5(P>0.05),but the BIS,VAS,and OAA/S scores of group M were lower than those of group L(P<0.05).The HR,RR,SpO_(2),MAP,and BIS of group H were lower than those of group L at T0-T5(P<0.05),and the VAS and OAA/S scores were also lower than those of group L(P<0.05).The HR,RR,SpO_(2),MAP,and BIS of group H were lower than those of group M at T0-T5(P<0.05),while there was no statistically significant difference in VAS and OAA/S scores between the two groups(P>0.05).There was a statistically significant difference in the patient satisfaction scores(M group>H group>L group)(P<0.05).Conclusion The application effect of 7.5μg/kg alfentanil combined with dexmedetomidine in percutaneous vertebroplasty demonstrates the optimal efficacy.
