摘　　要：目的调查分析首次驻守某热带海岛官兵疾病谱的构成特点,为进一步完善海岛医疗保障提供科学依据。方法选取某海岛医院2023年1—10月间就诊官兵的病历资料,按《ICD-10疾病诊断名称及代码目录》规定的名称记录并归类,统计各系统及各种具体疾病的发病例数,对首次与多次驻岛官兵各系统疾病的发病情况进行统计学分析。结果在6956份有效病例中,系统性疾病13大类,位于前3位的是骨科疾病2155例(30.98%)、呼吸内科疾病1350例(19.41%)、消化内科疾病999例(14.36%);具体疾病122种,位于前3位的是上呼吸道疾病809例(21.7%)、关节软组织损伤466例(12.14%)、胃肠炎410例(10.67%)。首次驻岛官兵骨科、皮肤科疾病以及生物伤的发生率明显高于多次驻岛官兵,口腔科和耳鼻喉科疾病的发生率明显低于多次驻岛官兵,差异均有统计学意义(均P<0.05)。结论根据某热带海岛呈现出不同的新的疾病谱,制订常见病多发病防治方案、对首次驻岛官兵制订针对性保障措施,是未来做好海岛医疗保障工作的重点。Objective To investigate the compositional characteristics of the disease spectrum of officers and soldiers stationed on a tropical island for the first time,and to provide a scientific basis for improving medical support on islands.Methods The medical records of patients treated at a hospital on an island from January to October 2023 were collected.The names of diseases were recorded and classified according to the International Statistical Classification of Disease and Pelated Health Proinlems,Tenth Revision(ICD-10),and the number of cases of diseases in each system and specific diseases were counted,respectively.Statistical analysis was conducted on the number of cases of diseases in various systems among the officers and soldiers stationed on a tropical island for the first time and multiple times.Results Among 6956 valid cases,systemic diseases were classified into 13 major categories.The top three categories were:orthopedic diseases(30.98%),respiratory diseases(19.41%),and gastrointestinal diseases(14.36%).A total of 122 specific diseases were identified,with the top three being:upper respiratory tract infections(21.7%),soft tissue injuries of joints(12.14%),and gastroenteritis(10.67%).The officers and soldiers stationed on a tropical island for the first time had a significantly higher incidences of orthopedic diseases,dermatological diseases,and traumatic injuries compared with those stationed for multiple times,and a significantly lower incidence of dental and otolaryngological diseases(all P<0.05).Conclusion Based on the special disease spectrum observed in a certain tropical island,formulating prevention and treatment plans for common diseases and frequently occurring diseases,as well as developing targeted health protection measures for the officers and soldiers stationed on a tropical island for the first time,will be key priorities for improving medical support on island in the future.
