高压氧联合芪苈强心胶囊治疗老年心力衰竭的疗效及对心功能、Galectin-3、TGF-β1的影响

摘　　要：目的探讨高压氧联合芪苈强心胶囊治疗老年心力衰竭的临床疗效及对心功能、半乳糖凝集素-3(Galectin-3)、转化生长因子-β1(TGF-β1)的影响。方法选取2022年1月至2024年1月山西白求恩医院老年科收治的老年心力衰竭患者108例,按照随机数字表法分为A、B、C 3组,每组36例。A组患者给予心力衰竭常规治疗;B组患者在A组治疗基础上联合芪苈强心胶囊口服治疗;C组患者在B组治疗基础上联合高压氧治疗。评估3组患者治疗前后的中医症状得分及临床疗效;治疗前后采用酶联免疫吸附法检测患者血清Galectin-3、TGF-β1和N末端脑钠肽前体(NTpro-BNP)水平,采用超声心动图仪检测患者每搏输出量(SV)、左室舒张末期内径(LVEDD)和左室射血分数(LVEF)等心功能指标水平,采用全自动生化分析仪检测血清尿素氮和肌酐水平。结果与治疗前比较,治疗后3组患者的中医症状得分均明显下降,且C组低于A、B组,差异均有统计学意义(均P<0.05);C组患者治疗总有效率为94.44%,明显高于B组(77.78%)和A组(63.89%),差异均有统计学意义(均P<0.05);与治疗前比较,治疗后3组患者血清Galectin-3、TGF-β1、NTpro-BNP水平均明显降低,且C组低于A、B组;SV、LVEF均明显升高,LVEDD、血清尿素氮、肌酐水平均明显下降,且C组改变幅度明显大于A、B组,差异均有统计学意义(均P<0.05)。结论高压氧联合芪苈强心胶囊治疗老年心力衰竭能改善中医症状,提高临床治疗效果,降低血清Galectin-3和TGF-β1、NTpro-BNP水平,改善心、肾功能。Objective To investigate the clinical efficacy of hyperbaric oxygen combined with Qiliqiangxin capsule in the treatment of senile heart failure and the effects on cardiac function,Galectin-3,and transforming growth factor-β1(TGF-β1).Methods A total of 108 elderly patients with heart failure admitted to the Geriatric Department of Shanxi Bethune Hospital from January 2022 to January 2024 were selected and divided into Group A,Group B,and Group C,with 36 cases in each,according to the random number table method.Group A received conventional treatment,Group B additionally received Qiliqiangxin capsule on the basis of the treatment of the Group A,and Group C additionally received hyperbaric oxygen therapy on the basis of the treatment of the Group B.The TCM symptom scores and clinical efficacy of the three groups were evaluated before and after treatment.The levels of Galectin-3,TGF-β1,and N-terminal brain natriuretic peptide(NTpro-BNP)were detected by enzyme-linked immunosorbent assay.Stroke volume(SV),left ventricular end-diastolic diameter(LVEDD),and left ventricular ejection fraction(LVEF)were measured by echocardiography.Serum urea nitrogen and creatinine levels were measured by automatic biochemical analyzer.Results After treatment,the scores of TCM symptoms all decreased in the three groups,and those in Group C were lower than those in Group A and B,all the differences were statistically significant(all P<0.05).The total effective rate of Group C(94.44%)was significantly higher than those of Group B(77.78%)and Group A(63.89%)(all P<0.05).Compared with those before treatment,the serum levels of Galectin-3,TGF-β1,and NTpro-BNP in the three groups significantly decreased after treatment,with Group C showing a more pronounced reduction than groups A and B.Additionally,SV and LVEF significantly increased,while LVEDD,serum urea nitrogen,and creatinine levels significantly decreased,with the magnitude of changes in Group C being significantly greater than in groups A and B.These differences were statistically sig
