老年糖尿病周围神经病变患者行高压氧辅助治疗时精细化护理的运用效果

摘　　要：目的探索老年糖尿病周围神经病变患者行高压氧辅助治疗时精细化护理的运用效果。方法前瞻性选取2021年3月至2023年3月秦皇岛市第一医院、秦皇岛港口医院、秦皇岛市中医院收治的126例老年糖尿病周围神经病变患者,根据随机数字表法分为2组,对照组63例采用常规护理,观察组63例采用精细化护理,均在护理后7 d比较效果。结果观察组护理后7 d的尺神经传导速度、腓总神经传导速度和正中神经传导速度均优于对照组[(57.97±3.25)m/s vs.(52.18±2.31)m/s,(45.69±3.21)m/s vs.(41.34±2.33)m/s,(54.88±4.11)m/s vs.(50.12±3.59)m/s,均P<0.05],护理后7 d的多伦多临床症状量表(TCSS)评分、临床神经检查量表(MDNS)评分均低于对照组[(4.12±1.76)分vs.(6.66±1.51)分,(6.21±2.11)分vs.(9.65±2.87)分,均P<0.05],护理后3 d足背动脉血流量、护理后7 d足背动脉血流量高于对照组[(30.55±1.36)ml/min vs.(28.53±1.85)ml/min,(33.54±1.17)ml/min vs.(30.43±1.22)ml/min,均P<0.05]。观察组患者对护理质量的满意度、护理态度满意度、周围环境满意度、人文关怀满意度均高于对照组(98.41%vs.87.30%,96.82%vs.82.54%,95.24%vs.80.95%,96.82%vs.82.54%,均P<0.05),住院期间并发症发生率低于对照组(3.17%vs.14.28%,P<0.05)。结论对行高压氧治疗的老年糖尿病周围神经病变患者实施精细化护理,不仅更能够促使病情恢复,还可提高护理满意度,降低住院期间并发症发生率。Objective To study the application effect of refined nursing in elderly diabetic patients with peripheral neuropathy during hyperbaric oxygen therapy.Methods A total of 126 cases of elderly patients with diabetic peripheral neuropathy admitted to The First Hospital of Qinhuangdao,Qinhuangdao Port Hospital,and Qinhuangdao Hospital of Traditional Chinese Medicine from March 2021 to March 2023 were retrospectively selected and divided into two groups,according to the random number table method.The 63 cases in the control group received routine care,while the 63 cases in the observation group received refined nursing care.The outcomes of nursing care were compared after a 7-day period of care.Results After 7 days of care,the ulnar nerve conduction velocity,common peroneal nerve conduction velocity,and median nerve conduction velocity in the observation group were superior to those in the control group[(57.97±3.25)m/s vs.(52.18±2.31)m/s,(45.69±3.21)m/s vs.(41.34±2.33)m/s,(54.88±4.11)m/s vs.(50.12±3.59)m/s,all P<0.05].After 7 days of care,the TCSS and MDNS scores were lower in the observation group compared with those in the control group[(4.12±1.76)points vs.(6.66±1.51)points,(6.21±2.11)points vs.(9.65±2.87)points,all P<0.05].The dorsal artery blood flows of the foot after 3 days and 7 days of care were all higher in the observation group than those in the control group[(30.55±1.36)mL/min vs.(28.53±1.85)mL/min,(33.54±1.17)mL/min vs.(30.43±1.22)mL/min,all P<0.05].Patient satisfaction with the quality of care,nursing attitude,surrounding environment,and humanistic care in the observation group were all higher than those in the control group(98.41%vs.87.30%,96.82%vs.82.54%,95.24%vs.80.95%,96.82%vs.82.54%,all P<0.05).The incidence of complications during hospitalization was lower in the observation group compared with that in the control group(3.17%vs.14.28%)(P<0.05).Conclusion Implementing refined nursing care for elderly patients with diabetic peripheral neuropathy undergoing hyperbaric oxygen therapy not
