基于“气归精,精归化”理论探讨代谢综合征肾损害的辨治思路

摘　　要：代谢综合征肾损害是由于多种有害代谢物质成分长期异常聚集沉积引起的肾脏功能障碍,能量代谢失常是其发病的重要原因。一方面,“气归精,精归化”理论体现了物质与能量相互转化的过程,物质的离散释放能量,此为“气归精”,能量的聚合化生物质,此为“精归化”;另一方面,“气归精,精归化”是指脾胃将摄入的食物中的精微物质转化为后天之精,再逐步通过脾胃传输到人体,维持人体的基本生命活动,其内涵为顾护脾胃,脾气健运则水谷及后天气血生化有源,气血足则肾气得以充盈,脾气充则清阳得以升,浊邪可去,此与代谢综合征肾损害的病机本质和主要治疗方向相合。Metabolic syndrome kidney damage is a renal dysfunction caused by the long-term abnormal accumulation and deposition of various harmful metabolic substances,and energy metabolism disorders are an important cause of its onset.The theory of“Qi returns to essence,essence returns to transformation”reflects the process of mutual conversion between matter and energy.The discrete release of energy from matter is called“Qi returns to essence”,and the aggregation of energy into biomass is called“essence returns to transformation”.On the other hand,it refers to the spleen and stomach converting the fine substances in the ingested food into acquired essence,which is gradually transported to the human body through the spleen and stomach to maintain the basic life activities of the body.Its connotation is to care for the spleen and stomach,maintain good spleen and stomach health,and activate blood biochemistry in the future.When Qi and blood are sufficient,kidney qi can be filled.When the temperament is full,clear yang can rise and turbid evil can be eliminated,which is consistent with the pathogenesis and main treatment direction of metabolic syndrome kidney damage.
