己酮可可碱注射液致严重肝功能异常1例

摘  要:患者,男,51岁,诊断:①2型糖尿病;②高血压病2级(极高危)。为改善循环,给予己酮可可碱注射液0.2 g,加入0.9%氯化钠注射液250 mL,静脉滴注,qd。治疗4 d后,肝功能:丙氨酸氨基转移酶2390.80 U·L^(-1),天冬氨酸氨基转移酶948.28 U·L^(-1),γ-谷氨酰转移酶517.81 U·L^(-1)。考虑己酮可可碱导致肝酶异常升高可能性较大,有明确的用药关联性。停用己酮可可碱注射液后,给予保肝降酶治疗,肝功能指标逐渐好转。临床使用己酮可可碱的过程中应加强用药监测,保障患者用药安全。A male patient,51 years old,was diagnosed as follows:①type 2 diabetes mellitus;②grade 2 hypertension(extremely high risk).To improve circulation,the patient was administered pentoxifylline injection 0.2 g mixed with 250 mL of 0.9%sodium chloride injection via an intravenous drip,once daily(qd).After 4 days of treatment,liver function tests showed the following results:alanine aminotransferase(ALT)at 2390.80 U·L^(-1),aspartate aminotransferase(AST)at 948.28 U·L^(-1),and gamma-glutamyltransferase(GGT)at 517.81 U·L^(-1).It was highly probable that pentoxifylline caused abnormally elevated liver enzymes,with a clear drug-related association.After discontinuing pentoxifylline injection and initiating liver-protecting and enzyme-lowering treatment,the liver function indicators gradually improved.Enhanced monitoring during the clinical use of pentoxifylline is essential to ensure patient safety.
