夫妻疾病沟通干预在乳腺癌患者及其配偶照顾者中的应用

摘　　要：目的:分析基于治疗性沟通系统(TCS)的夫妻疾病沟通干预方案对乳腺癌患者及配偶照顾者的应用效果。方法:选取2023年6月1日-12月1日在某院乳甲外科就诊的80对乳腺癌患者及配偶照顾者作为研究对象,将他们随机分为干预组(n=40)和对照组(n=40)。对照组采用乳甲外科常规护理;干预组在常规护理的基础上,接受为期3个化疗周期基于TCS的夫妻疾病沟通干预方案,比较两组乳腺癌患者及配偶照顾者干预前后夫妻间疾病沟通情况[夫妻癌症相关沟通问题量表(CRCP量表)]、癌症复发恐惧[恐惧疾病进展简化量表(FoP-Q-SF)]及希望水平[Herth希望量表(HHI)]。结果:干预后,干预组乳腺癌患者的CRCP及FoP-Q-SF总分分别低于对照组(Z=-6.157,-4.481;P<0.05),但HHI总分高于对照组(Z=4.810,P<0.05);干预组乳腺癌配偶照顾者的CRCP及FoP-Q-SF总分分别低于对照组(Z=-5.770,-5.831;P<0.05),但HHI总分高于对照组(Z=2.269,P<0.05)。结论:基于治疗性沟通系统的夫妻疾病沟通干预方案能有效促进乳腺癌患者及配偶照顾者间疾病相关问题的沟通,降低癌症复发恐惧水平,提高希望水平。Objective:To analyze the therapeutic communication system based on marital disease communication intervention program for breast cancer patients and spouse caregivers.Methods:A total of 80 pairs of breast cancer pa-tients and their spouse caregivers who were treated in the breast surgery department of a hospital from June 1,2023 to December 1,2023 were selected as research objects,and they were randomly divided into the intervention group(n=40)and the control group(n=40).The control group was treated with routine nursing.In addition to conventional care,the intervention group received a three-cycle TCs-based disease communication intervention program for couples.The disease communication between spouses of the two groups of breast cancer patients and their spouse caregivers before and after the intervention were compared[Marital cancer-related Communication Problems Scale(CRCP scale)],fear of cancer recurrence[Simplified Fear of Disease Progression Scale(FoP-Q-SF)]and hope level[Herth Hope Scale(HHI)].Results:After intervention,the total scores of CRCP and FoP-Q-SF in the intervention group were lower than those in the control group(Z=-6.157,-4.481;P<0.05),but the total scores of HHI were higher than those in the control group(Z=4.810,P<0.05).The total scores of CRCP and FoP-Q-SF in the intervention group were lower than those in the control group(Z=-5.770,-5.831;P<0.05),but the total scores of HHI were higher than those in the control group(Z=2.269,P<0.05).Conclusion:The therapeutic communication system can effectively promote the communication of disease-related issues between breast cancer patients and their spouse caregivers,reduce the level of fear of cancer recurrence,and increase the level of hope.
