输尿管软镜碎石术后患者发生尿路感染的影响因素

摘　　要：目的:分析输尿管软镜碎石术后患者发生尿路感染的影响因素。方法:回顾性分析2021年2月至2024年2月于该院进行输尿管软镜碎石术的145例输尿管结石患者的临床资料,根据术后是否发生尿路感染将其分为发生组(n=33)和未发生组(n=112),统计两组基线资料,采用Logistic回归分析输尿管软镜碎石术后患者发生尿路感染的影响因素。结果:两组年龄、性别、结石直径、合并糖尿病、术后尿管留置时间、手术时间比较,差异均有统计学意义(P<0.05);两组体质量指数、结石部位、结石位置、结石梗阻类型、吸烟史、饮酒史、合并高血压、术前有无使用排石药物、住院时间比较,差异均无统计学意义(P>0.05);Logistic回归分析结果显示,年龄>60岁、女性、结石直径≥1.5 cm、术后尿管留置时间≥3 d、手术时间≥60 min均为输尿管软镜碎石术后患者发生尿路感染的危险因素(OR>1,P<0.05)。结论:年龄>60岁、女性、结石直径≥1.5 cm、术后尿管留置时间≥3 d、手术时间≥60 min均为输尿管软镜碎石术后患者发生尿路感染的危险因素。Objective:To analyze influencing factors of urinary tract infection in patients after flexible ureteroscopic lithotripsy.Methods:The clinical data of 145 patients with ureteral calculi who underwent flexible ureteroscopic lithotripsy in this hospital from February 2021 to February 2024 were retrospectively analyzed.According to whether postoperative infection occurred,they were divided into the occurrence group(n=33)and the non-occurrence group(n=112).The baseline data of the two groups were statistically analyzed.Logistic regression was used to analyze the influencing factors of urinary tract infection in the patients after flexible ureteroscopic lithotripsy.Results:There were significant differences in age,sex,stone diameter,combined diabetes,postoperative indwelling time of urinary tube and operation time between the two groups(P<0.05).There was no statistically significant difference in body mass index,stone location,stone obstruction type,smoking history,alcohol consumption history,combined hypertension,preoperative use of stone expelling drugs,and hospitalization time between the two groups(P>0.05).The Logistic regression analysis showed that age>60 years old,female,stone diameter≥1.5 cm,postoperative catheterization time≥3 d,and operation time≥60 minutes were all risk factors for urinary tract infection in the patients undergoing ureteroscopic lithotripsy(OR>1,P<0.05).Conclusions:Age>60 years old,female,stone diameter≥1.5 cm,postoperative catheter indwelling time≥3 d,and operation time≥60 min are the risk factors for urinary tract infection in the patients after flexible ureteroscopic lithotripsy.
