龙氏整脊手法联合颈椎牵引治疗寰枢关节脱位患者的效果

摘　　要：目的:观察龙氏整脊手法联合颈椎牵引治疗寰枢关节脱位患者的效果。方法:回顾性分析2023年3月至2024年3月该院收治的70例寰枢关节脱位患者的临床资料,根据治疗方法不同将其分为对照组和观察组各35例。对照组采用颈椎牵引治疗,观察组在对照组基础上联合龙氏整脊手法治疗,比较两组临床疗效,治疗前后寰枢关节指标(寰椎齿突轴线差值、齿突侧块间距差值)水平和生命质量(Barthel指数)评分。结果:观察组治疗总有效率为97.14%(34/35),高于对照组的74.29%(26/35),差异有统计学意义(P<0.05);治疗后,两组寰椎齿突轴线差值、齿突侧块间距差值均小于治疗前,且观察组小于对照组,差异有统计学意义(P<0.05);治疗后,两组Barthel指数评分均高于治疗前,且观察组高于对照组,差异有统计学意义(P<0.05)。结论:龙氏整脊手法联合颈椎牵引治疗寰枢关节脱位可提高治疗总有效率和生命质量评分,减小寰枢关节指标水平,效果优于单纯颈椎牵引治疗。Objective:To observe effects of Long’s chiropractic manipulation combined with cervical traction in treatment of patients with atlantoaxial dislocation.Methods:The clinical data of 70 patients with atlantoaxial dislocation admitted to this hospital from March 2023 to March 2024 were retrospectively analyzed.According to different treatment methods,they were divided into control group and observation group,35 cases in each group.The control group was treated with cervical traction,while the observation group was treated with Long’s chiropractic manipulation on the basis of that of the control group.The clinical efficacy the atlantoaxial joint index(atlanto-odontoid axis difference,odontoid lateral mass spacing difference)levels and the quality of life(Barthel index)scores were compared between the two groups before and after the treatment.Results:The total effective rate of the observation group was 97.14%(34/35),which was higher than 74.29%(26/35)of the control group,and the difference was statistically significant(P<0.05).After the treatment,the levels of atlanto-odontoid axis difference and odontoid lateral mass spacing difference in the two groups were smaller than those before the treatment,those in the observation group were smaller than those in the control group,and the differences were statistically significant(P<0.05).Further,after the treatment,the Barthel index scores of the two groups were higher than those before the treatment,that in the observation group was higher than that in the control group,and the differences were statistically significant(P<0.05).Conclusions:Long’s chiropractic manipulation combined with cervical traction in the treatment of atlantoaxial dislocation can improve the total effective rate and the quality of life scores,and reduce the levels of atlantoaxial joint indexes.Moreover,it is superior to simple cervical traction treatment.
