摘  要:目的探讨血清微小核糖核酸(miRNA)-192、miRNA-210、骨形态发生蛋白(BMP)-2和BMP-7表达与股骨颈骨折术后骨折延迟愈合的关系及预测价值。方法选择股骨颈骨折患者183例作为研究对象,按照患者是否出现术后骨折延迟愈合分为正常组(n=147)与延迟组(n=36)。比较两组血清miRNA-192和miRNA-210相对表达量和血清BMP-2、BMP-7水平;采用Pearson分析miRNA-192、miRNA-210与BMP-2、BMP-7相关性;采用多因素logistic回归分析影响骨折延迟愈合独立危险因素;采用受试者工作特征(ROC)曲线分析miRNA-192、miRNA-210、BMP-2和BMP-7对骨折延迟愈合的预测价值。结果延迟组血清miRNA-192相对表达量、血清BMP-2和BMP-7水平均低于正常组,而miRNA-210相对表达量高于正常组(t分别=16.69、19.75、12.02、-22.49,P均<0.05)。经Pearson分析显示,miRNA-192与BMP-2和BMP-7呈正相关(r分别=0.67、0.55,P均<0.05),而miRNA-210与BMP-2和BMP-7呈负相关(r分别=-0.52、-0.48,P均<0.05)。经多因素logistic回归分析显示,miRNA-192、miRNA-210、BMP-2和BMP-7为影响骨折延迟愈合独立危险因素(OR分别=8.01、5.84、17.34、4.20,P均<0.05)。经ROC曲线分析显示,miRNA-192、miRNA-210、BMP-2和BMP-7预测骨折延迟愈合的曲线下面积分别为0.94、0.95、0.89、0.89。结论血清miRNA-192、miRNA-210、BMP-2、BMP-7水平对股骨颈骨折术后患者骨折延迟愈合具有良好预测价值。Objective To explore the relationship and predictive value of serum microRNA(miRNA-192),miRNA 210,bone morphogenetic protein-2(BMP-2),and BMP-7 expressions with delayed fracture healing after femoral neck fracture surgery.Methods Totally 183 patients with femoral neck fractures were selected as the study subjects and divided into a normal group(n=147)and a delayed group(n=36)based on whether they experienced delayed healing after surgery.The relative expression levels of miRNA-192 and miRNA-210 in serum and the levels of BMP-2 and BMP-7 in serum between two groups were compared.Pearson analysis was used to investigate the correlation between miRNA 192,miRNA-210 and BMP-2,BMP-7.Multiple logistic regression analysis was used to identify independent risk factors affecting delayed fracture healing.The receiver operating characteristic(ROC)curves was used to analyze the predictive value of miRNA-192,miRNA-210,BMP-2,and BMP-7 for delayed fracture healing.Results The delayed group relative expression levels of miRNA-192,BMP-2,and BMP-7 were lower than those of normal group,while the relative expression level of miRNA-210 was higher than that of normal group(t=16.69,19.75,12.02,-22.49,P<0.05).Pearson analysis showed that miRNA-192 was positively correlated with BMP-2 and BMP-7(r=0.67,0.55,P<0.05),while miRNA-210 was negatively correlated with BMP-2 and BMP-7(r=-0.52,-0.48,P<0.05).Multivariate logistic regression analysis showed that miRNA-192,miRNA-210,BMP-2,and BMP-7 were independent risk factors affecting delayed fracture healing(OR=8.01,5.84,17.34,4.20,P<0.05).According to ROC curve analysis,the predicted areas under the curves for miRNA 192,miRNA-210,BMP-2,and BMP-7 predicting delayed fracture healing were 0.94,0.95,0.89,and 0.89,respectively.Conclusion miRNA-192,miRNA-210,serum BMP-2,and BMP-7 levels have good predictive value for delayed fracture healing in patients with femoral neck fractures after surgery.
