摘 要:目的构建患者统一随访平台,提升随访效率,减少系统对接,改善患者体验。方法改革随访管理模式,依托人工智能技术实现包含患者信息、随访任务管理、随访数据接入管理和随访数据分析四大功能模块的随访平台。结果系统实施后,随访患者59917例,随访成功率为89.07%;加入慢病服务的患者同期增长42.55%,科研入组患者同期增长9.37%,差异有统计学意义(P<0.05)。结论患者统一随访平台能有效提升随访效率、规范系统接入标准、实现患者分流和改善患者体验。Objective To establish a unified follow-up platform for patients,enhancing follow-up efficiency,reducing system integration,and improving patient experience.Methods Reform the followup management model by leveraging artificial intelligence technology to create a follow-up platform that includes four major functional modules:patient information,follow-up task management,follow-up data integration management,and follow-up data analysis.Results After the implementation of the system,59,917 patients were followed up,with a follow-up success rate of 89.07%.The number of patients joining chronic disease services increased by 42.55%during the same period,and the number of patients enrolled in research studies increased by 9.37%during the same period,with the differences being statistically significant(P<0.05).Conclusion The unified follow-up platform for patients can effectively enhance followup efficiency,standardize system integration,facilitate patient triage,and improve patient experience.
