摘  要:目的探讨基于计划-实施-检查-行动(PDCA)循环的改良鸟巢式管理模式干预对早产儿院内感染发生率及生长发育的影响,为改善早产儿预后提供依据。方法回顾性收集医院收治的100例早产儿临床资料,按护理管理方法不同分为对照组和PDCA鸟巢组各50例,分别接受常规管理和基于PDCA的改良鸟巢式管理。统计并比较2组早产儿在医院内的感染发生率,以及家属的管理满意度;分别在入组时和管理6周后,收集2组早产儿的生长发育情况(身长、体重、头围),采用新生儿神经行为测查法(NBNA)评估早产儿的神经发育情况。结果PDCA鸟巢组早产儿的院内感染发生率(2.00%)低于对照组(18.00%),家属的管理满意度(98.00%)高于对照组(84.00%),差异有统计学意义(χ^(2)=7.111、5.983,均P<0.05)。管理6周后,PDCA鸟巢组的身长、体重、头围增长幅度、NBNA评分均大于对照组,差异有统计学意义(P<0.05)。结论基于PDCA的改良鸟巢式管理模式,可降低早产儿的院内感染发生率,促进早产儿的身体生长和神经发育,并提高家属的满意度。Objective To explore the impact of an improved bird's nest management model intervention based on the Plan-Do-Check-Act(PDCA)cycle on the incidence of nosocomial infections and growth and development in premature infants,and to provide evidence for improving the prognosis of premature infants.Methods Clinical data of 100 premature infants admitted to Henan Children's Hospital Zhengzhou Children's Hospital from June 2023 to March 2024 were retrospectively collected.These infants were categorized into a control group and a PDCA bird's nest group based on different nursing methods,with 50 cases in each group.They received conventional management and improved bird's nest style management based on PDCA,respectively.The incidence of nosocomial infections in two groups of premature infants and the satisfaction of their families with management were statistically compared.The growth and development(length,weight,head circumference)of the two groups of premature infants were collected at the time of enrollment and after 6 weeks of management.The neonatal behavioral neurological assessment(NBNA)was used to assess the neurodevelopment of the premature infants.Results The incidence of nosocomial infections in premature infants in the PDCA bird's nest group(2.00%)was lower than that in the control group(18.00%),and the satisfaction of family members with management(98.00%)was higher than that in the control group(84.00%),with statistically significant differences(χ^(2)=7.111,5.983,both P<0.05).After 6 weeks of management,the growth in body length,weight,head circumference,and NBNA scores in the PDCA bird's nest group were all greater than those in the control group,with statistically significant differences(P<0.05).Conclusions The improved bird's nest management model based on PDCA can reduce the incidence of nosocomial infections in premature infants,promote their physical growth and neurological development,and enhance family satisfaction.
