针刺十三鬼穴联合黄连阿胶汤对围绝经期失眠患者神经递质、性激素水平及主客观睡眠质量的影响

摘　　要：目的:研究针刺十三鬼穴联合黄连阿胶汤在围绝经期失眠患者中的应用价值。方法:将110例围绝经期失眠患者,根据不同治疗方法分为甲组(35例)、乙组(35例)、丙组(40例)。甲组予常规西医疗法,乙组在甲组治疗基础上予黄连阿胶汤加减方治疗,丙组在乙组基础上予针刺十三鬼穴疗法治疗。比较各组的主客观睡眠质量、神经递质、性激素水平。结果:治疗1个月后及治疗后3个月时,血清促卵泡生成素、促黄体生成素水平比较,丙组<乙组<甲组(P<0.05),雌二醇水平比较,丙组>乙组>甲组(P<0.05)。治疗3个月后,各组总卧床时间比较,差异无统计学意义(P>0.05);总睡眠时间、睡眠效率、5-羟色胺(5-hydroxytryptamine,5-HT)、β-内啡肽(β-endorphin),β-EP)水平比较,丙组>乙组>甲组(P<0.05);睡眠潜伏期、觉醒次数、5-羟吲哚乙酸(5-hydroxyindole acetic acid,5-HIAA)水平比较,丙组<乙组<甲组(P<0.05)。结论:针刺十三鬼穴联合黄连阿胶汤应用于围绝经期失眠患者,可改善患者性激素、神经递质水平,进而提高睡眠质量。Objective:To study the application values of needling at thirteen ghost points joined with Huanglian Ejiao decoction in the treatment of perimenopausal insomnia.Methods:All 110 patients were allocated to A group(35 cases),B group(35 cases)and C group(40 cases)according to different therapeutic methods.A group accepted conventional therapy of Western medicine,B group took modified Huanglian Ejiao decoction based on the therapy of A group,and C group accepted needling at thirteen ghost points based on the treatment that B group accepted.To compare subjective and objective sleep quality,the levels of neurotransmitter and sex hormone between three groups.Results:When serum follicle-stimulating hormone(FSH)and luteinizing hormone(LH)were compared between three groups at one month and three months after the treatment,C group<B group<A group(P<0.05),the comparisons of the levels of estradiol showed that C group>B group>A group(P<0.05).The difference had no statistical meaning in the total time of resting in bed between all the groups at one month after the treatment(P>0.05);the comparisons of overall sleep time,sleep efficiency,the levels of 5-HT andβ-EP revealed that C group>B group>A group(P<0.05);the comparisons of sleep latency,waking times and the levels of 5-HIAA displayed that C group<B group<A group(P<0.05).Conclusion:The application of needling at thirteen ghost points joined with Huanglian Ejiao decoction in the treatment of perimenopausal insomnia is good for the improvement of the levels of sex hormone and neurotransmitter,thereby improving sleep quality.
