基于真实世界“寒热温凉并用法”治疗非小细胞肺癌的临床评价研究

摘　　要：目的:在真实世界中研究以“寒热温凉并用法”为指导思想,形成的“道医养药文化流派———肺癌专方”治疗非小细胞肺癌(NSCLC)的临床有效性及安全性。方法:根据患者的真实治疗情况,回顾性纳入“肺癌专方”治疗组66例和对照组66例临床资料。对照组在成都中医药大学附属医院先接受铂类药物加细胞毒类药物化疗,化疗完成后,口服非“肺癌专方”中药免煎颗粒治疗24周;试验组使用“肺癌专方”治疗24周。结果:试验组和对照组一年生存率比较,无显著差异。对照组实体瘤有效率显著高于试验组(P<0.05);试验组和对照组实体瘤控制率比较,无显著差异。试验组临床中重度胃肠道反应发生率、肝功能损害率、肾功能损害率、中重度白细胞减少率显著低于对照组(P<0.05)。结论:真实世界临床研究的结果显示:单独使用“肺癌专方”能够较好地控制NSCLC实体瘤,并且产生严重毒副反应的发生率显著低于对照组患者,一年生存率与化疗加免煎颗粒的治疗方案并无显著差异。这显示了“寒热温凉并用法”治疗NSCLC“有效控制、带瘤生存”的中医特色,为中医药治疗肺癌拓宽思路,临床应用价值较高。Objective:The article aims to explore the clinical efficacy and safety of“Special Formular for Lung Cancer”based on“Cold,Hot,Warm,and Cool”Using together Therapy strategy in the treatment of Non-small-cell lung cancer(NSCLC)patients.Methods:66patients were included in the“Special Formular for Lung Cancer”treatment group and 66patients in the control group in this retrospective real world study.The control group received non-“Special Formular for Lung Cancer”&non-fried granule treatment after chemotherapy for 24weeks.The treatment group patients received“Special Formular for Lung Cancer”alone for 24weeks.Results:There was no significant difference between the two groups with respect to the one-year sur-vival rate.The effective rate of the control group in tumor size was significantly higher than the treatment group.There was no sig-nificant difference between the two groups in the control rate in tumor size.The incidence of middle-to-severe gastrointestinal re-actions,the incidence of liver function impairment,the incidence of renal function impairment,and the reduction rate of middle-to-severe white blood cell of the treatment group were significantly lower than the control group.Conclusions:The results demon-strated that using“Special Formular for Lung Cancer”alone can effectively bring tumor under control and the incidence of severe adverse effects was significantly lower than chemotherapy.Importantly,there was no significant difference in one-year survival be-tween“Special Formular for Lung Cancer”and chemotherapy.This showed the advantage of“control tumors&survive with tumors”based on the“Cold,Hot,Warm,and Cool”Using together Therapy strategy.We hope our study can expand treatment approaches with respect to Traditional Chinese Medicine for the management of lung cancer.
