摘　　要：目的:基于免疫探讨异功散治疗慢性扁桃体炎的疗效及机制。方法:选取120例气虚痰瘀型慢性扁桃体炎患者,随机分为对照组和观察组各60例。对照组运用扁桃体烙治法治疗,每周2次,观察组在此基础上加用异功散加味口服,每日1剂。2组均以1周为1个疗程,连续治疗4个疗程。对比两组患者治疗前后症状体征积分、中医证候积分、血清炎性因子(IFN-γ、TNF-α、IL-4)水平及血清T淋巴细胞亚群水平。结果:治疗组总有效率高于对照组(P<0.05)。治疗后,2组症状体征积分及中医证候积分均降低(P<0.05),且观察组低于对照组(P<0.05);2组血清IFN-γ水平较治疗前升高,TNF-α、IL-4水平较前下降,观察组改善效果优于对照组(P<0.05);观察组外周血T淋巴细胞CD4^(+)水平及CD4^(+)/CD8^(+)的提高优于对照组(P<0.05)。结论:异功散加味能有效改善气虚痰瘀型慢性扁桃体炎患者的临床症状,并能改善患者血清IFN-γ、TNF-α、IL-4表达水平及更好地调节T淋巴细胞亚群,调节患者的免疫功能。Objective:To explore the efficacy and mechanism of Yigong powder in the treatment of chronic tonsillitis based on immunity.Methods:120patients with chronic tonsillitis of qi deficiency and phlegm stasis type were randomly divided into con-trol group and observation group,60cases in each group.The control group was treated with tonsil cauterization,twice a week.On this basis,the observation group was treated with modified Yigong Powder,one dose per day.Both groups were treated for 4consecutive courses of treatment,with 1week as a course of treatment.The scores of symptoms and signs,TCM syndromes,ser-um inflammatory factors(IFN-γ,TNF-α,IL-4)and serum T lymphocyte subsets were compared between the two groups before and after treatment.Results:The total effective rate of the treatment group was higher than that of the control group(P<0.05).After treatment,the scores of symptoms and signs and TCM syndromes in the two groups were all(P<0.05),and the observation group was lower than the control group(P<0.05).The level of serum IFN-γwas higher than that before treatment,and the level of TNF-α,IL-4was lower than that before treatment in the two groups.The improvement effect of the observation group was better than that of the control group(P<0.05).The levels of CD4^(+)and CD4^(+)/CD8^(+)of T lymphocytes in the observation group were higher than those in the control group(P<0.05).Conclusion:Modified Yigong Powder can effectively improve the clinical symp-toms of patients with chronic tonsillitis of qi deficiency and phlegm stasis type,improve the expression levels of serum IFN-γ,TNF-αand IL-4,better regulate T lymphocyte subsets,and regulate the immune function of patients.
