疏肝健脾法联合西药治疗消化性溃疡疗效与安全性的Meta分析

摘　　要：目的对疏肝健脾法联合西药治疗消化性溃疡的有效性及安全性进行评价。方法运用计算机检索中国知网、维普、万方、CBM、PubMed等数据库,纳入符合标准的文献,使用Review Manager 5.4软件进行Meta分析。结局指标包括临床有效率、溃疡愈合情况、Hp根除率、血清炎症因子、不良反应及复发率。结果共纳入17篇文献,1677例患者。Meta分析结果显示,与单用西药治疗比较,疏肝健脾法联合西药治疗消化性溃疡可提高临床有效率(OR=4.67,95%CI:3.24~6.72,Z=8.27,P<0.00001),促进溃疡愈合(OR=2.36,95%CI:1.13~4.92,P=0.02),提高Hp根治率(OR=2.82,95%CI:1.37~5.83,P=0.005),降低血清炎症因子水平较强且不良反应较小,复发率较低。结论与单用西药比较,疏肝健脾法联合西药治疗消化性溃疡疗效更优,不良反应较小,复发率较低。由于纳入研究的文献总体质量一般,有效性和安全性亟待设计严谨的高质量临床随机对照试验加以验证。Objective Evaluating the therapeutic effect and safety of the soothing liver and strengthening spleen method combined with western medicine on the treatment of peptic ulcer.Methods Searching CNKI,VIP,Wanfang,CBM,PubMed and other databases by computer and including literatures that meet the criteria.Meta-analysis was performed using Review Manager 5.4 software.Outcome indicators included clinical eff ective rate,quality of ulcer healing,Hp eradication rate,serum infl ammatory factor,adverse reactions and recurrence rate.Results A total of 17 literatures were included,with a total of 1837 patients.The results of meta-analysis showed that compared with western medicine alone,the combination of soothing the liver and strengthening the spleen can improve the clinical effi ciency of peptic ulcer(OR=4.67,95%CI:3.24-6.72,Z=8.27,P<0.00001),promote ulcer healing(OR=2.36,95%CI:1.13-4.92,P=0.02),improve Hp cure rate(OR=2.82,95%CI:1.37-5.83,P=0.005),reduce serum infl ammatory factor levels and the adverse reaction is small,recurrence rate is low.Conclusion Compared with western medicine alone,the combination of soothing the liver and strengthening the spleen has better curative eff ect,less adverse reactions and lower recurrence rate.Due to the general quality of the literature included in the study,the eff ectiveness and safety need to be verifi ed by well-designed high-quality clinical randomized controlled trial.
