PGC-1β、HIF-1α、RETN在痛风性关节炎中的表达水平及与关节破坏程度的相关性分析

摘　　要：目的探讨过氧化物酶体增殖物激活受体γ共激活因子-1β(PGC-1β)、缺氧诱导因子-1α(HIF-1α)、抵抗素(RETN)在痛风性关节炎(GA)中的表达水平,并分析与关节破坏程度的相关性。方法选取2022年1月至2023年10月该院收治的134例GA患者为GA组,另选取同期在该院进行健康体检的134例体检健康者为对照组,比较两组血清PGC-1β、HIF-1α、RETN表达水平。比较GA组不同临床特征患者血清、滑膜液PGC-1β、HIF-1α、RETN表达水平,关节破坏程度按照主诉疼痛程度分级法评分(VAS)分为重度GA亚组78例,轻度GA亚组56例,比较不同关节破坏程度PGC-1β、HIF-1α、RETN、骨破坏因子[β交联降解产物(β-CTX)、抗酒石酸酸性磷酸酶-5b(TRACP5b)、核因子κB受体活化因子配体(RANKL)]、炎症因子[肿瘤坏死因子-α(TNF-α)、白细胞介素-1β(IL-1β)]表达水平,分析血清、滑膜液PGC-1β、HIF-1α、RETN与关节破坏程度、骨破坏因子、炎症因子的关联性。结果GA组血清PGC-1β表达水平低于对照组,血清HIF-1α、RETN表达水平高于对照组(P<0.05);不同临床分期、受累关节、病程及年发作频率GA患者血清、滑膜液PGC-1β、HIF-1α、RETN表达水平比较,差异均有统计学意义(P<0.05);重度GA亚组血清、滑膜液PGC-1β表达水平低于轻度GA亚组(P<0.05),HIF-1α、RETN表达水平高于轻度GA亚组(P<0.05);重度GA亚组β-CTX、TRACP5b表达水平高于轻度GA亚组(P<0.05),RANKL表达水平低于轻度GA亚组(P<0.05);血清、滑膜液PGC-1β与关节破坏程度、β-CTX、TRACP5b、TNF-α、IL-1β呈负相关,与RANKL呈正相关,HIF-1α、RETN与关节破坏程度、β-CTX、TRACP5b、TNF-α、IL-1β呈正相关,与RANKL呈负相关。结论PGC-1β、HIF-1α、RETN在GA患者中异常表达,且与关节破坏程度、骨破坏因子及炎症因子关系密切,有望成为评估GA发生、进展的可靠指标。Objective To investigate the expression levels of peroxisome proliferator-activated receptor gamma coactivator-1β(PGC-1β),hypoxia-inducible factor-1α(HIF-1α),and resistin(RETN)in gouty arthritis(GA),and analyze their correlation with the degree of joint damage.Methods A total of 134 patients with GA in the hospital from January 2022 to October 2023 were selected as GA group,and 134 healthy people who underwent the physical examination in the same period were selected as control group.The serum expression levels of PGC-1β,HIF-1αand Retn were compared between the two groups.The expression levels of PGC-1β,HIF-1αand Retn in serum and synovial fluid of patients with different clinical characteristics in GA group were compared,the degree of joint destruction was graded according to the subjective pain grading method(VAS)and the patients were divided into a severe GA subgroup of 78 cases and a mild GA subgroup of 56 cases.The expression levels of PGC-1β,HIF-1α,RETN,bone destruction factors[β-cross linked degradation products(β-CTX),tartrate resistant acid phosphatase-5b(TRACP5b),receptor activator of nuclear factor kappa B ligand(RANKL)]and inflammatory factors[tumor necrosis factor-α(TNF-α),interleukin-1β(IL-1β)]were compared among different degrees of joint destruction,and the correlation between serum and synovial fluid PGC-1β,HIF-1α,RETN and the degree of joint destruction,bone destruction factors,and inflammatory factors was analyzed.Results The expression level of PGC-1βin serum of GA group was lower than that of the control group,while the expression levels of HIF-1αand RETN in serum were higher than those of the control group(P<0.05).There were statistically significant differences in serum and synovial fluid PGC-1β,HIF-1α,and RETN levels among GA patients with different clinical stages,affected joints,disease duration,and annual seizure frequency(P<0.05).The expression levels of PGC-1βin the serum and synovial fluid of the severe GA subgroup were lower than those of the mild GA subgroup(
