营养支持小组联合透析中卧式脚踏运动对慢性肾衰维持性血液透析患者透析充分性及营养状况的影响

摘　　要：目的:探讨营养支持小组联合透析中卧式脚踏运动在慢性肾衰维持性血液透析(MHD)患者中的应用效果。方法:选取2022年1月—2024年1月河南省直第三人民医院收治的148例慢性肾衰MHD患者作为研究对象,采用随机数表法分为对照组(n=74)和观察组(n=74)。对照组行常规护理,观察组行营养支持小组联合透析中卧式脚踏运动。对比两组患者透析充分性、营养状况、并发症发生率、生活质量及患者满意度。结果:干预后,观察组患者尿素下降率(URR)、尿素清除指数(Kt/V)均高于对照组,差异均有统计学意义(t=8.517、6.549,P<0.05);观察组患者营养状况评分均低于对照组,差异均有统计学意义(t=7.851、8.011、9.570、9.262,P<0.05);观察组患者并发症总发生率低于对照组,差异有统计学意义(χ^(2)=4.132,P<0.05);观察组患者健康调查问卷(SF-36)评分均高于对照组,差异均有统计学意义(t=9.956、8.745、7.292、7.283、7.683、8.388、7.463、7.047,P<0.05);观察组患者满意度高于对照组,差异有统计学意义(χ^(2)=4.132,P<0.05)。结论:营养支持小组联合透析中卧式脚踏运动可改善慢性肾衰MHD患者的营养状况,增强透析充分性,减少并发症,提高生活质量及患者满意度。Objective:To investigate the effect of nutrition support group combined with horizontal foot movement in dialysis in patients with chronic renal failure maintenance hemodialysis(MHD).Methods:148 MHD patients with chronic renal failure admitted to the hospital from January 2022 to January 2024 were selected and divided into two groups according to random number table method,with 74 cases in each group.The control group received routine nursing,and the observation group received nutrition support group combined with horizontal foot exercise in dialysis.The adequacy of dialysis,nutritional status,incidence of complications,quality of life and patient satisfaction were compared between the two groups.Results:After intervention,urea reduction rate(URR)and urea clearance index(Kt/V)in observation group were higher than those in control group(t=8.517,6.549;P<0.05).Nutritional status score of observation group was lower than that of control group(t=7.851,8.011,9.570,9.262;P<0.05).The complication rate in the observation group was lower than that in control group(χ^(2)=4.132,P<0.05).SF-36 scores of observation group were higher than control group,and the difference was statistically significant(t=9.956,8.745,7.292,7.283,7.683,8.388,7.463,7.047;P<0.05).The satisfaction of the patients in the observation group was higher than that in the control group,and the difference was statistically significant(χ^(2)=4.132,P<0.05).Conclusion:Nutrition support group combined with horizontal foot movement during dialysis can improve the nutritional status of MHD patients with chronic renal failure,enhance dialysis adequacy,reduce complications,and improve quality of life and patient satisfaction.
