血清sFlt-1/PLGF比值、Hcy水平与妊娠期高血压疾病孕妇妊娠结局的关系

摘　　要：目的分析血清可溶性酪氨酸激酶-1(sFlt-1)/胎盘生长因子(PLGF)比值、同型半胱氨酸(Hcy)水平与妊娠期高血压疾病孕妇妊娠结局的关系。方法选择2023年4月至2024年2月在该院确诊为妊娠期高血压疾病的90例孕妇作为观察组,依据妊娠期高血压疾病孕妇妊娠结局分为良好妊娠结局组和不良妊娠结局组。另外选择同期于该院产检正常的70例孕妇作为对照组。比较观察组和对照组血清sFlt-1、PLGF、Hcy水平及sFlt-1/PLGF比值,比较不同病情严重程度妊娠期高血压疾病孕妇血清sFlt-1、PLGF、Hcy水平及sFlt-1/PLGF比值,比较不同妊娠结局妊娠期高血压疾病孕妇血清sFlt-1、PLGF、Hcy水平及sFlt-1/PLGF比值。绘制受试者工作特征(ROC)曲线分析sFlt-1、PLGF、Hcy及sFlt-1/PLGF比值预测妊娠期高血压疾病孕妇不良妊娠结局的价值。结果观察组血清sFlt-1、Hcy水平及sFlt-1/PLGF比值均高于对照组(P<0.05),PLGF水平低于对照组(P<0.05)。妊娠期高血压组血清sFlt-1、Hcy水平及sFlt-1/PLGF比值均低于子痫前期组和重度子痫前期组(P<0.05),PLGF水平高于子痫前期组和重度子痫前期组(P<0.05);子痫前期组血清sFlt-1、Hcy水平及sFlt-1/PLGF比值均低于重度子痫前期组(P<0.05),PLGF水平高于重度子痫前期组(P<0.05)。良好妊娠结局组28例,不良妊娠结局组62例。不良妊娠结局组血清sFlt-1、Hcy水平及sFlt-1/PLGF比值均高于良好妊娠结局组(P<0.05),PLGF水平低于良好妊娠结局组(P<0.05)。ROC曲线分析结果显示,sFlt-1、PLGF、Hcy及sFlt-1/PLGF比值预测妊娠期高血压疾病孕妇不良妊娠结局的曲线下面积(AUC)分别为0.785、0.779、0.716及0.915。结论血清Hcy水平及sFlt-1/PLGF比值与妊娠期高血压疾病孕妇的病情严重程度及妊娠结局均有关,临床诊治时可结合以上指标进行判断。Objective To analyze the relationship between the serum soluble fms-like tyrosine kinase-1(sFlt-1)/placental growth factor(PLGF)ratio and homocysteine(Hcy)level with the pregnant outcome in pregnant women with gestational hypertension disorders.Methods Ninety pregnant women with diagnosed gestational hypertension disorders in this hospital from April 2023 to February 2024 were selected as the observation group and divided into the good pregnancy outcome group and poor pregnancy outcome group according to the pregnant outcome of pregnant women with hypertensive disorders during pregnant period.The other 70 pregnant women with normal antenatal examination in this hospital during the same period were selected as the control group.The serum sFlt-1,PLGF and Hcy levels and the sFlt-1/PLGF ratio were compared between the observation group and control group.The serum sFlt-1,PLGF and Hcy levels and the sFlt-1/PLGF ratio were compared among different condition severities of the pregnant women with hypertensive disorders during pregnant period.The levels of serum sFlt-1,PLGF,Hcy and the sFlt-1/PLGF ratio were compared among the pregnant women with different pregnancy outcomes of hypertensive disorders.The receiver operating characteristic(ROC)curve was drawn to analyze the value of sFlt-1,PLGF,Hcy and sFlt-1/PLGF ratio in predicting the poor pregnancy outcome of pregnant women with gestational hypertensive disorders.Results The serum sFlt-1 and Hcy levels and sFlt-1/PLGF ratio in the observation group were all higher than those in the control group(P<0.05),and the PLGF level was lower than that in the control group(P<0.05).The serum sFlt-1 and Hcy levels and sFlt-1/PLGF ratio in the pregnant hypertensive disorder group were all lower than those in the preeclampsia group and severe preeclampsia group(P<0.05),and the PLGF level was higher than that in the preeclampsia group and severe preeclampsia group(P<0.05);the serum sFlt-1 and Hcy levels and Flt-1/PLGF ratio in the preeclampsia group were all lower than those in the seve
