摘  要:目的探究甲状腺良性结节热消融术后体积缩小率(VRR)特征及其影响因素。方法在2018年1月至2022年8月,选取了60例在本院接受热消融术治疗甲状腺良性结节患者,并对其进行了为期1年的随访研究。根据结节VRR特征,患者被分为两组,分别为预后良好组36例、预后不良组24例。对两组的临床数据、治疗前后的甲状腺结节体积以及并发症进行了详细比较,通过运用logistic回归模型,进一步分析热消融术后影响良性结节预后不良的因素。结果两组患者性别、年龄、结节数量比较,差异无统计学意义(P>0.05);预后良好组的结节囊性比例高于预后不良组,结节实质性和混合性比例低于预后不良组;预后良好组的结节处于安全区比例高于预后不良组,结节处于危险区比例低于预后不良组;预后良好组的结节体积小于预后不良组,单位消融时间少于预后不良组,差异有统计学意义(P<0.05)。预后良好组甲状腺结节的VRR在术后1个月、3个月、6个月及12个月均高于预后不良组,差异有统计学意义(P<0.05)。logistic回归分析结果显示,结节囊实质性或混合性[OR=3.807,95%CI(1.913~7.576)]、结节位置处于危险区[OR=3.814,95%CI(1.082~8.072)]、结节体积越大[OR=21.286,95%CI(4.248~106.660)]及单位体积消融时间越长[OR=1.950,95%CI(1.013~3.754)]是热消融术后良性结节发生预后不良的影响因素。结论甲状腺良性结节热消融术后体积缩小率与结节性质是否为囊实质性或混合性、结节位置是否处于危险区、结节体积及单位体积消融时间有关。Objective To explore the characteristics of the volume reduction rate(VRR)after thermal ablation of benign thyroid nodules and its influencing factors.Methods Between January 2018 and August 2022,60 patients with benign thyroid nodules who underwent thermal ablation at our hospital were selected for a 1-year follow-up study.Based on the VRR characteristics of the nodules,patients were divided into two groups:a good prognosis group(36 cases)and a poor prognosis group(24 cases).Detailed comparisons were made between the clinical data,nodule volumes before and after treatment,and complications of the two groups.Logistic regression analysis was conducted to further explore the factors influencing poor prognosis following thermal ablation of benign nodules.Results There were no statistically significant differences in gender,age,or nodule number between the two groups(P>0.05).The proportion of cystic nodules was higher in the good prognosis group compared to the poor prognosis group,while the proportions of solid and mixed nodules were lower in the good prognosis group.The proportion of nodules located in the safe zone was higher in the good prognosis group,whereas the proportion in the dangerous zone was lower.Additionally,nodule volumes were smaller in the good prognosis group,and the ablation time per unit volume was less than in the poor prognosis group,with statistically significant differences(P<0.05).The VRR of thyroid nodules in the good prognosis group at 1,3,6,and 12 months post-operation was higher than that in the poor prognosis group,with statistical significance(P<0.05).Logistic regression analysis showed that nodules being solid or mixed[OR=3.807,95%CI(1.913~7.576)],located in the dangerous zone[OR=3.814,95%CI(1.082–8.072)],larger nodule volume[OR=21.286,95%CI(4.248~106.660)],and longer ablation time per unit volume[OR=1.950,95%CI(1.013~3.754)]were significant factors influencing poor prognosis after thermal ablation.Conclusion The volume reduction rate of benign thyroid nodules after thermal ablation
