摘 要:目的探讨以心肺运动为指导的运动康复治疗对冠心病(CHD)患者的影响,为临床提供参考。方法回顾性分析2022年1月至2023年12月郑州大学附属郑州中心医院收治的120例CHD患者的临床资料,根据治疗方案的不同分为对照组(常规治疗)和研究组(常规治疗+心肺运动指导的运动康复治疗),各60例。比较两组患者心肌标志物水平、心肺功能指标、6 min步行试验(6MWT)结果及生活质量评分。结果治疗后,两组患者肌酸激酶同工酶(CKMB)、氨基末端脑钠肽前体(NT-proBNP)水平降低,且研究组低于对照组,差异有统计学意义(P<0.05)。治疗后,两组患者的峰值摄氧量(VO_(2)peak)、VO_(2)peak占预计值的百分比、无氧域、峰值代谢当量(METs)升高,且研究组高于对照组,差异有统计学意义(P<0.05)。治疗后,两组患者的6MWT距离增加,且研究组高于对照组,差异有统计学意义(P<0.05)。治疗后,两组患者简明健康状况调查问卷(SF-36)评分升高,且研究组高于对照组,差异有统计学意义(P<0.05)。结论对CHD患者予以心肺运动指导的运动康复治疗干预,可减轻其心肌损伤程度,改善心肺功能,提升运动能力,改善生活质量。Objective To explore the impact of cardiopulmonary exercise-guided rehabilitation therapy on patients with coronary heart disease(CHD),providing a reference for clinical practice.Methods A retrospective analysis was conducted on the clinical data of 120 CHD patients admitted to Zhengzhou Central Hospital affiliated to Zhengzhou University from January 2022 to December 2023.Patients were divided into a control group(n=60,routine treatment)and an study group(n=60,routine treatment plus cardiopulmonary exercise-guided rehabilitation therapy)based on treatment plans.Comparisons were made between the two groups in levels of myocardial markers,cardiopulmonary function indices,6-minute walk test(6MWT)results,and quality of life scores.Results Post-treatment,levels of creatine kinase-MB(CK-MB)and N-terminal pro-brain natriuretic peptide(NT-proBNP)decreased in both groups,with the study group showing greater reductions(P<0.05).The peak oxygen uptake(VO_(2)peak),percentage of predicted VO_(2)peak,anaerobic threshold,and peak metabolic equivalents(METs)increased in both groups post-treatment,with the study group exhibiting more significant improvements(P<0.05).The 6MWT distance increased after treatment in both groups,with the study group achieving higher distances(P<0.05).Additionally,scores on the Short Form-36(SF-36)health survey increased in both groups,with the study group showing higher scores(P<0.05).Conclusion Cardiopulmonary exercise-guided rehabilitation intervention for CHD patients can reduce myocardial injury,improve cardiopulmonary function,enhance exercise capacity,and elevate quality of life.
