身痛逐瘀汤配合温针灸治疗腰椎间盘突出症疼痛的临床研究

摘　　要：目的分析身痛逐瘀汤联合温针灸治疗腰椎间盘突出症(气滞血瘀型)疼痛的临床效果。方法选择在三门峡市中医院2022年3月至2024年2月期间诊治的80例腰椎间盘突出症患者为研究对象,按照随机数表分为对照组与观察组,各40例,均为患者进行常规治疗,对照组实施温针灸,观察组在温针灸基础上联合身痛逐瘀汤,分析经过1个月的治疗后患者的、Oswestry功能障碍指数(ODI)、视觉模拟评分法(VAS)评分、腰椎日本骨科学会(JOA)评分、白细胞介素-1β(IL-1β)、神经肽Y(NPY)水平变化情况。结果治疗前组间指标和评分对比差异无统计学意义(P>0.05),治疗后,观察组的ODI、VAS评分低于对照组,观察组的JOA评分明显提升,且高于对照组,治疗后观察组的IL-1β、NPY水平均低于对照组,组间对比差异均有统计学意义(P<0.05)。结论身痛逐瘀汤联合温针灸有利于减轻腰椎间盘突出症患者的疼痛程度,进一步改善患者的腰椎功能。Objective To analyze the clinical effect of Shentong Zhuyu Decoction combined with warm acupuncture in treating pain caused by lumbar disc herniation of the Qi stagnation and blood stasis type.Methods Eighty patients with lumbar disc herniation diagnosed and treated at Sanmenxia Central Hospital from March 2022 to February 2024 were selected for the study.They were randomly divided into a control group and an observation group,with 40 patients in each group,all receiving routine treatment.The control group received warm acupuncture,while the observation group received Shentong Zhuyu Decoction in addition to warm acupuncture.After one month of treatment,changes in the Oswestry Disability Index(ODI),Visual Analogue Scale(VAS)scores,Japanese Orthopaedic Association(JOA)lumbar scores,interleukin-1β(IL-1β),and neuropeptide Y(NPY)levels were analyzed.Results Before treatment,there were no statistically significant differences between groups in terms of indicators and scores(P>0.05).After treatment,the observation group showed lower ODI and VAS scores compared to the control group.The JOA scores in the observation group increased significantly and were higher than those in the control group.Post-treatment IL-1βand NPY levels in the observation group were lower than those in the control group,with statistically significant differences between groups(P<0.05).Conclusion Shentong Zhuyu Decoction combined with warm acupuncture is beneficial in reducing the pain severity in patients with lumbar disc herniation and further improves lumbar function.
