摘 要:目的探讨布地奈德混悬液联合特布他林雾化治疗慢性阻塞性肺疾病急性加重期(AECOPD)的效果及对嗜酸性粒细胞水平的影响。方法选取庆阳市中医医院2022年1月至2024年9月收治的108例AECOPD患者,随机分为单药组与联合组,各54例。单药组患者采用布地奈德混悬液雾化吸入治疗,联合组在单药组基础上加用硫酸特布他林雾化液雾化治疗。比较两组临床疗效,治疗前后嗜酸性粒细胞水平、肺功能及血气指标变化,不良反应发生率。结果联合组总有效率90.74%高于单药组75.93%(P<0.05);两组治疗前外周血、痰液诱导嗜酸性粒细胞水平比较无明显差异(P>0.05),治疗后两组外周血、诱导痰液嗜酸性粒细胞水平均降低(P<0.05),与单药组相比,联合组均更低(P<0.05);两组治疗前用力肺活量(FVC)、第一秒钟用力呼气容积占用力肺活量比例(FEV_(1)/FVC)以及每分钟最大通气量(MVV)肺功能相关指标,动脉血氧分压(PaO_(2))、动脉血二氧化碳分压(PaCO_(2))相关血气指标比较无明显差异(P>0.05),治疗后两组FVC、FEV_(1)/FVC、MVV、PaO_(2)水平均升高(P<0.05),与单药组相比,联合组均更高(P<0.05),治疗后两组PaCO_(2)水平均降低(P<0.05),与单药组相比,联合组更低(P<0.05);两组均无明显不良反应发生,不良反应发生率比较无差异(P>0.05)。结论AECOPD治疗中采用布地奈德混悬液联合特布他林雾化的效果显著,可改善患者外周血及痰液中嗜酸性粒细胞水平,提高通气功能与肺功能,且安全性较高。Objective To explore the effect of budesonide suspension combined with terbutaline nebulization in the treatment of acute exacerbation of chronic obstructive pulmonary disease(AECOPD)and its impact on eosinophil levels.Methods A total of108 patients with AECOPD admitted to Qingyang Hospital of Traditional Chinese Medicine from January 2022 to September 2024were randomly divided into monotherapy group and combination group,with 54 cases in each group.The single drug group was treated with budesonide suspension aerosol inhalation,and the combined group was treated with terbutaline sulfate aerosol aerosol on the basis of the single drug group.The clinical efficacy,the changes of eosinophil level,lung function and blood gas index before and after treatment,and the incidence of adverse reactions were compared between the two groups.Results The total effective rate of combination group was 90.74%higher than that of monotherapy group 75.93%(P<0.05).There were no significant differences in eosinophils induced in peripheral blood and sputum between the two groups before treatment(P>0.05).After treatment,eosinophils induced in peripheral blood and sputum decreased(P<0.05),and compared with the monotherapy group,the combined group was lower(P<0.05).There were no significant differences between the two groups before treatment in forced vital capacity(FVC),the proportion of forced expiratory volume in the first second to forced vital capacity(FEV_(1)/FVC),the maximum volume per minute(MVV),and the blood gas indexes related to arterial partial pressure of oxygen(PaO_(2))and arterial partial pressure of carbon dioxide(PaCO_(2))(P>0.05).After treatment,the levels of FVC,FEV_(1)/FVC,MVV and PaO_(2) in both groups were increased(P<0.05),compared with the single drug group,the combined group were higher(P<0.05),the PaCO_(2) levels in both groups were decreased(P<0.05),compared with the single drug group,the combined group was lower(P<0.05);There was no significant adverse reaction between the two groups,and there was no significant
