初产妇剖宫产意愿的影响因素

摘　　要：目的:分析初产妇剖宫产意愿的影响因素。方法:选取2022年5月至2023年7月于该院待产的1360名初产妇进行横断面研究,观察1360名初产妇的分娩方式意愿,根据分娩方式意愿将其分为剖宫产组和自然分娩组。比较两组一般资料,采用多因素Logistic回归分析初产妇剖宫产意愿的影响因素。结果:1360名初产妇中,503名分娩方式意愿为剖宫产,剖宫产意愿率为36.99%(503/1360);857名分娩方式意愿为自然分娩。剖宫产组文化程度大专及以上、家庭月平均收入≥10 000元/月、有手术史、分娩恐惧程度为高水平、产妇母亲建议剖宫产等占比均高于自然分娩组,差异有统计学意义(P<0.05)。Logistic回归分析结果显示,文化程度大专及以上、家庭月平均收入≥10 000元/月、分娩恐惧程度为高水平、产妇母亲建议剖宫产等均为初产妇剖宫产意愿的危险因素(OR>1,P<0.05)。结论:文化程度大专及以上、家庭月平均收入≥10 000元/月、分娩恐惧程度为高水平、产妇母亲建议剖宫产等均为初产妇剖宫产意愿的危险因素。Objective:To analyze influencing factors of cesarean section willingness of primiparas.Methods:A cross-sectional study was conducted on 1360 primiparas who were waiting for delivery in the hospital from May 2022 to July 2023.The willingness of delivery mode of the 1360 primiparas was observed,and they were divided into cesarean section group and natural delivery group according to the willingness of delivery mode.The general data of the two groups were compared.Multivariate Logistic regression analysis was used to analyze the influencing factors for the willingness of the primiparas to cesarean section.Results:Among the 1360 primiparas,503 were willing to give birth by cesarean section,and the willingness rate of cesarean section was 36.99%(503/1360).857 women were willing to give birth naturally.The proportions of primiparas with the junior college education and above,average monthly household income≥10,000 yuan/month,history of surgery,high level of fear of childbirth,and maternal mother’s suggestion of cesarean section in the cesarean section group were higher than those in the natural delivery group,and the differences were statistically significant(P<0.05).Logistic regression analysis showed that education level of junior college and above,family monthly average income≥10,000 yuan/month,high level of fear of childbirth,and maternal mother’s suggestion of cesarean section were all risk factors for primipara’s willingness to cesarean section(OR>1,P<0.05).Conclusions:The education level of junior college and above,the average monthly household income≥10,000 yuan/month,the high level of fear of childbirth,and the maternal mother’s suggestion of cesarean section are the risk factors for the willingness of the primiparas to cesarean section.
