康柏西普联合玻璃体切割术治疗增殖期糖尿病性视网膜病变患者的效果

摘　　要：目的:观察康柏西普联合玻璃体切割术治疗增殖期糖尿病性视网膜病变患者的效果。方法:回顾性分析2020年11月至2021年11月郑州华厦眼科医院收治的150例增殖期糖尿病性视网膜病变患者的临床资料,按照治疗方法不同将其分为观察组(n=80)与对照组(n=70)。对照组采用玻璃体切割术治疗,观察组在对照组基础上联合康柏西普治疗。比较两组手术前后患眼最佳矫正视力、黄斑中心凹厚度、眼压、生化指标[血清补体C1q肿瘤坏死因子相关蛋白9(CTRP9)、Delta样配体4(DLL4)、脂肪特异性丝氨酸蛋白酶抑制剂(Vaspin)]水平,以及并发症发生率。结果:术后1、3个月,两组患眼最佳矫正视力水平均低于术前,且观察组低于对照组,差异有统计学意义(P<0.05);术后1、3个月,两组黄斑中心凹厚度均小于术前,且观察组小于对照组,差异有统计学意义(P<0.05);术后1、3个月,两组眼压水平均低于术前,且观察组低于对照组,差异有统计学意义(P<0.05);术后,两组CTRP9水平均高于术前,且观察组高于对照组,两组DLL4、Vaspin水平均低于术前,且观察组低于对照组,差异有统计学意义(P<0.05);两组并发症发生率比较,差异无统计学意义(P>0.05)。结论:康柏西普联合玻璃体切割术治疗增殖期糖尿病性视网膜病变患者可改善生化指标水平,降低患眼最佳矫正视力水平、黄斑中心凹厚度和眼压水平,效果优于单纯玻璃体切割术治疗。Objective:To observe effects of Conbercept combined with vitrectomy in treatment of patients with proliferative diabetic retinopathy.Methods:The clinical data of 150 patients with proliferative diabetic retinopathy admitted to Zhengzhou Huaxia Eye Hospital from November 2020 to November 2021 were retrospectively analyzed.According to different treatment methods,they were divided into observation group(n=80)and control group(n=70).The control group was treated with vitrectomy,while the observation group was treated with Conbercept on the basis of that of the control group.The best corrected visual acuity,the macula central foveal thickness,the intraocular pressure,the biochemical indexes[serum complement C1q tumor necrosis factor-related protein 9(CTRP9),Delta-like ligand 4(DLL4),visceral adipose tissue-derived serine protease inhibitor(Vaspin)]before and after surgery,and the incidence of complications were compared between the two groups.Results:1 and 3 months after the surgery,the best corrected visual acuity levels of the two groups were lower than those before the surgery,those in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).1 and 3 months after the surgery,the thickness of macula central fovea in the two groups was smaller than that before the surgery,a that in the observation group was smaller than that in the control group,and the differences were statistically significant(P<0.05).1 and 3 months after the surgery,the intraocular pressure levels of the two groups were lower than those before the surgery,that in the observation group was lower than that in the control group,and the differences were statistically significant(P<0.05).After the surgery,the levels of CTRP9 in the two groups were higher than those before the surgery,and that in the observation group was higher than that in the control group;the levels of DLL4 and Vaspin in the two groups were lower than those before the surgery,and those in the observation group were low
