血清25(OH)D、Hcy、AMH水平联合检测对多囊卵巢综合征不孕症患者IVF-ET妊娠失败的预测价值

摘　　要：目的:分析血清25-羟维生素D[25(OH)D]、同型半胱氨酸(Hcy)、抗米勒管激素(AMH)水平联合检测对多囊卵巢综合征不孕症患者体外受精-胚胎移植(IVF-ET)妊娠失败的预测价值。方法:回顾性分析2021年3月至2023年3月于该院接受IVF-ET治疗的164例多囊卵巢综合征不孕症患者的临床资料,根据是否妊娠将其分为妊娠成功组和妊娠失败组,比较两组血清25(OH)D、Hcy、AMH水平,并绘制受试者工作特征(ROC)曲线,分析其单项及联合检测对多囊卵巢综合征不孕症患者IVF-ET妊娠失败的预测价值。结果:妊娠失败组血清25(OH)D、AMH水平均低于妊娠成功组,血清Hcy水平高于妊娠成功组,差异有统计学意义(P<0.05);ROC曲线分析结果显示,血清25(OH)D、Hcy、AMH水平单项及联合检测预测多囊卵巢综合征不孕症患者IVF-ET妊娠失败的曲线下面积分别为0.750、0.766、0.769、0.888,联合检测预测价值高于三者单项检测。结论:血清25(OH)D、Hcy、AMH水平联合检测对多囊卵巢综合征不孕症患者IVF-ET妊娠失败的预测价值高于三者单项检测。Objective:To analyze predictive value of combined detection of serum 25-hydroxyvitamin D[25(OH)D],homocysteine(Hcy)and anti-Müllerian hormone(AMH)levels for in vitro fertilization-embryo transfer(IVF-ET)pregnancy failure in infertile patients with polycystic ovary syndrome.Methods:The clinical data of 164 infertile patients with polycystic ovary syndrome who received IVF-ET treatment in this hospital from March 2021 to March 2023 were retrospectively analyzed.They were divided into pregnancy success group and pregnancy failure group according to whether they were pregnant.The levels of serum 25(OH)D,Hcy and AMH were compared between the two groups.The receiver operating characteristic(ROC)curve was drawn to analyze the predictive value of single and combined detection for IVF-ET pregnancy failure in the infertile patients with polycystic ovary syndrome.Results:The levels of serum 25(OH)D and AMH in the pregnancy failure group were lower than those in the successful pregnancy group,the serum Hcy level was higher than that in the successful pregnancy group,and the differences were statistically significant(P<0.05).The ROC curve analysis showed that the area under the curve of serum 25(OH)D,Hcy and AMH levels for predicting IVF-ET pregnancy failure in the infertile patients with polycystic ovary syndrome were 0.750,0.766,0.769 and 0.888,separately;and the predictive value of combined detection was higher than that of single detection.Conclusions:The predictive value of combined detection of serum 25(OH)D,Hcy and AMH levels for IVF-ET pregnancy failure in the infertile patients with polycystic ovary syndrome is higher than that of the three single tests.
