关节镜下微创内固定术与切开复位内固定术治疗胫骨平台骨折患者的效果比较

摘　　要：目的:比较关节镜下微创内固定术与切开复位内固定术治疗胫骨平台骨折患者的效果。方法:回顾性分析2022年3月至2024年3月该院收治的92例胫骨平台骨折患者的临床资料,按照手术方式不同将其分为对照组(n=49)和研究组(n=43)。对照组采用切开复位内固定术治疗,研究组采用关节镜下微创内固定术治疗。比较两组手术相关指标(手术切口长度、手术时间、首次下床活动时间、住院时间、骨折愈合时间、术中出血量)水平、手术前后膝关节功能(Lysholm功能评分)评分和术后并发症发生率。结果:研究组手术切口长度、手术时间、首次下床活动时间、住院时间、骨折愈合时间均短于对照组,术中出血量少于对照组,差异有统计学意义(P<0.05);术后6个月,两组Lysholm功能评分均高于术前,且研究组高于对照组,差异有统计学意义(P<0.05);研究组术后并发症发生率为6.97%(3/43),低于对照组的24.49%(12/49),差异有统计学意义(P<0.05)。结论:关节镜下微创内固定术治疗胫骨平台骨折患者可提高膝关节功能评分,缩短康复进程,降低并发症发生率的效果优于切开复位内固定术治疗。Objective:To compare effects of arthroscopic minimally invasive internal fixation and open reduction and internal fixation in treatment of patients with tibial plateau fractures.Methods:The clinical data of 92 patients with tibial plateau fractures admitted to this hospital from March 2022 to March 2024 were retrospectively analyzed.According to different surgical methods,they were divided into control group(n=49)and study group(n=43).The control group was treated with open reduction and internal fixation,while the study group was treated with arthroscopic minimally invasive internal fixation.The levels of operation related indexes(incision length,operation time,first out-of-bed time,hospitalization time,fracture healing time,intraoperative blood loss),the knee joint function(Lysholm function score)scores before and after the surgery,and the incidence of postoperative complications were compared between the two groups.Results:The incision length,the operation time,the first out-ofbed time,the hospitalization time and the fracture healing time of the study group were shorter than those of the control group,the intraoperative blood loss was less than that of the control group,and the differences were statistically significant(P<0.05).Six months after the surgery,the Lysholm function scores of the two groups were higher than those before the surgery,that in the study group was higher than that in the control group,and the differences were statistically significant(P<0.05).Further,the incidence of postoperative complications in the study group was 6.97%(3/43),which was lower than 24.49%(12/49)in the control group,and the difference was statistically significant(P<0.05).Conclusions:Arthroscopic minimally invasive internal fixation for the treatment of the patients with tibial plateau fractures can improve the knee function scores,shorten the rehabilitation process,and reduce the incidence of complications.Moreover,it is superior to open reduction and internal fixation.
