左炔诺孕酮宫内节育系统与地屈孕酮治疗子宫内膜息肉术后患者的效果比较

摘　　要：目的:比较左炔诺孕酮宫内节育系统与地屈孕酮治疗子宫内膜息肉术后患者的效果。方法:回顾性分析2021年3月至2023年5月该院收治的120例子宫内膜息肉患者的临床资料,根据治疗方法不同将其分为对照组和观察组各60例。对照组给予地屈孕酮治疗,观察组给予左炔诺孕酮宫内节育系统治疗。比较两组治疗前后月经量[月经失血图(PBAC)]评分、子宫内膜厚度、性激素指标[孕酮、雌二醇(E_(2))]水平和血管内皮生长因子(VEGF)水平,不良反应发生率和1年复发率。结果:治疗后,两组PBAC评分均低于治疗前,且观察组低于对照组,子宫内膜厚度均小于治疗前,且观察组小于对照组,差异有统计学意义(P<0.05);两组孕酮、E_(2)、VEGF水平均高于治疗前,且观察组高于对照组,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05);观察组1年复发率为5.00%(3/60),低于对照组的16.67%(10/60),差异有统计意义(P<0.05)。结论:左炔诺孕酮宫内节育系统治疗子宫内膜息肉术后患者可提高性激素指标水平和VEGF水平,减少子宫内膜厚度,以及降低PBAC评分和复发率的效果优于地屈孕酮治疗效果。Objective:To compare effects of Levonorgestrel intrauterine system and Dydrogesterone in treatment of postoperative patients with endometrial polyps.Methods:The clinical data of 120 patients with endometrial polyps admitted to this hospital from March 2021 to May 2023 were retrospectively analyzed.According to different treatment methods,they were divided into control group and observation group,60 cases in each group.The control group was treated with Dydrogesterone,while the observation group was treated with Levonorgestrel intrauterine system.The menstrual volume[pictorial blood loss assessment chart(PBAC)]scores,the endometrial thickness,the sex hormone index[progesterone,estradiol(E_(2))]levels,the vascular endothelial growth factor(VEGF)levels before and after the treatment,the incidence of adverse reactions,and the 1-year recurrence rate were compared between the two groups.Results:After the treatment,the PBAC scores of the two groups were lower than those before the treatment,that in the observation group was lower than that in the control group,the endometrial thickness of the two groups were smaller than those before the treatment,that in the observation group was smaller than that in the control group,and the differences were statistically significant(P<0.05).After the treatment,the levels of progesterone,E_(2) and VEGF in the two groups were higher than those before the treatment,those in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).The 1-year recurrence rate of the observation group was 5.00%(3/60),which was lower than 16.67%(10/60)of the control group,and the difference was statistically significant(P<0.05).Conclusions:Levonorgestrel intrauterine system in the treatment of the postoperative patients with endometrial polyps can improve the level of sex hormone index and VEGF levels,and reduce the PBAC scores,the endome
