基于发展性照顾理念的全程无缝衔接护理在新生儿高胆红素血症患儿中的应用效果

摘　　要：目的:观察基于发展性照顾理念的全程无缝衔接护理在新生儿高胆红素血症(NHB)患儿中的应用效果。方法:选取2021年1月至2023年5月该院收治的112例NHB患儿进行前瞻性研究,按照随机数字表法将其分为对照组和研究组各56例。对照组实施常规护理,研究组实施基于发展性照顾理念的全程无缝衔接护理。比较两组护理前后总胆红素(TBIL)和脉搏血氧饱和度(SpO_(2))水平,每日摄奶量、黄疸消退时间、住院时间,并发症发生率,以及家长满意度评分。结果:护理后,两组TBIL水平均低于护理前,且研究组低于对照组,两组SpO_(2)水平均高于护理前,且研究组高于对照组,差异有统计学意义(P<0.05);研究组每日摄奶量多于对照组,黄疸消退时间、住院时间均短于对照组,差异有统计学意义(P<0.05);研究组并发症发生率为5.36%(3/56),低于对照组的17.86%(10/56),差异有统计学意义(P<0.05);研究组健康教育、护理操作、沟通交流、并发症预防评分等家长满意度评分均高于对照组,差异有统计学意义(P<0.05)。结论:基于发展性照顾理念的全程无缝衔接护理应用于NHB患儿中,可降低TBIL水平和并发症发生率,提高SpO_(2)水平和家长满意度评分,促进康复,效果优于常规护理。Objective:To observe application effects of whole-course seamless nursing based on developmental care concept in children with neonatal hyperbilirubinemia(NHB).Methods:A prospective study was conducted on 112 children with NHB admitted to the hospital from January 2021 to May 2023.According to the random number table method,they were divided into control group and study group,56 cases in each group.The control group was given routine nursing,while the study group was given the whole-course seamless nursing based on the developmental care concept.The levels of total bilirubin(TBIL)and pulse blood oxygen saturation(SpO_(2))before and after the nursing,the daily milk intake,the jaundice regression time,the hospitalization time,the incidence of complications,and the parental satisfaction score were compared between the two groups.Results:After the nursing,the TBIL levels of the two groups were lower than those before the nursing,and that in the study group was lower than that in the control group;the SpO_(2) levels of the two groups were higher than those before the nursing,and that in the study group was higher than that in the control group;and the differences were statistically significant(P<0.05).The daily milk intake of the study group was more than that of the control group,the jaundice regression time and hospitalization time were shorter than those of the control group,and the differences were statistically significant(P<0.05).The incidence of complications in the study group was 5.36%(3/56),which was lower than 17.86%(10/56)in the control group,and the difference was statistically significant(P<0.05).Further,the scores of parental satisfaction such as health education,nursing operation,communication and complication prevention in the study group were higher than those in the control group,and the differences were statistically significant(P<0.05).Conclusions:The whole-course seamless nursing based on the developmental care concept in the children with NHB can reduce the levels of TBIL and the incidence of co
