摘  要:目的探讨动态平衡测试期间膝骨关节炎(knee osteoarthritis,KOA)患者区别于健康受试者的脑电特征,研究大脑活动和KOA之间的联系和影响。方法15名KOA患者和17名健康受试者进行起立行走测试(timed up and go test,TUGT)和5次坐立测试(five times sit-to-stand,5STS),记录受试者大脑皮层中央区C_(Z)、C_(3)、C_(4),顶叶P_(Z)、P_(3)、P_(4)和枕叶O_(Z)、O_(1)、O_(2)这9个电极δ(0.5~4 Hz)、θ(4~8 Hz)、α(8~14 Hz)、β(14~30 Hz)4个频段的脑电特征,并进行时域和频域分析。结果KOA患者进行5STS时,δ频段C_(4)、P_(3)、P_(4)电极和θ频段C_(4)、P_(3)、P_(4)电极以及β频段P_(3)、P_(4)电极的时域幅值明显高于健康受试者(P<0.05);β频段P_(3)、P_(4)和O_(2)电极的脑电信号功率明显高于健康受试者。而KOA患者在进行TUGT时,除θ频段P_(Z)电极的时域幅值明显比健康受试者低(P<0.05),其余频段的各个电极与健康受试者相比均无显著性差异;θ频段C_(3)、C_(Z)、C_(4)、P_(Z)、P_(4)、O_(1)、O_(Z)、O_(2)电极和α频段C_(Z)、P_(3)、P_(Z)和O_(Z)电极以及β频段C_(Z)、P_(4)、O_(Z)和O_(2)电极的脑电信号功率明显比健康受试者低(P<0.05)。结论与健康受试者相比,在KOA患者进行5STS期间,大脑顶叶区域的活动更强,而在TUGT期间,KOA患者的中央区、顶叶和枕叶区域的大脑活动则比健康受试者弱。研究结果可为临床医生对KOA的预防和治疗提供一定参考。Objective To explore the electroencephalography(EEG)characteristics of patients with knee osteoarthritis(KOA)which are different from those of healthy subjects during dynamic balance tests,and study the connection and influence between brain activity and KOA.Methods Fifteen patients with KOA and 17 healthy subjects performed the timed up and go test(TUGT)and five times sit-to-stand test(5STS).The EEG characteristics of the 4 frequency bands,namelyδ(0.5-4 Hz),θ(4-8 Hz),α(8-14 Hz)andβ(14-30 Hz)of 9 electrodes including C_(Z),C_(3),C_(4) in the central region,P_(Z),P_(3),P_(4) in the parietal lobe,and O_(Z),O_(1),O_(2) in the 271 occipital lobe of the cerebral cortex of the subjects were recorded.Time and frequency domain analysis were conducted.Results During 5STS,the time-domain amplitudes of electrodes C_(4),P_(3),and P_(4) in theδband,electrodes C_(4),P_(3),and P_(4) in theθband,and electrodes P_(3) and P_(4) in theβband of KOA patients were significantly higher than those of healthy subjects(P<0.05);the power of EEG signals of electrodes P_(3),P_(4),and O_(2) in theβband was significantly higher than that of healthy subjects.During TUGT,except that the time-domain amplitude of electrode P_(Z) in theθband of KOA patients was significantly lower than that of healthy subjects(P<0.05),there was no significant difference in all electrodes in other frequency bands;the power of EEG signals of electrodes C_(3),C_(Z),C_(4),P_(Z),P_(4),O_(1),O_(Z),O_(2) in theδband and electrodes C_(Z),P_(3),P_(Z),and O_(Z) in theαband and electrodes C_(Z),P_(4),O_(Z),and O_(2) in theβband was significantly lower than that of healthy subjects(P<0.05).Conclusions Compared with healthy subjects,the activity in the parietal lobe area of the brain of KOA patients during 5STS was stronger,while the brain activity in the central region,parietal lobe,and occipital lobe areas of KOA patients during TUGT was weaker than that of healthy subjects.This study may provide some references for clinicians in the prevention and treatment of KO
