眶内电针治疗动眼神经麻痹临床疗效观察

摘　　要：目的观察眶内电针治疗动眼神经麻痹的疗效及患者在治疗过程中的恢复规律。方法回顾整理2020年1月—2023年6月在哈尔滨医科大学附属第一医院眼球运动障碍治疗中心接受眶内电针治疗的动眼神经麻痹患者的临床资料,对患者的动眼神经症状评分并进行疗效分析,观察动眼神经麻痹患者在治疗过程中的恢复规律,对影响痊愈的相关因素进行分析。结果共纳入221例动眼神经麻痹患者,痊愈患者51例,痊愈率为23.077%,有效、显效及痊愈患者共186例,总体有效率为84.163%,患者眼运动神经麻痹各项评分均明显降低,差异具有统计学意义(P<0.05),其中眶内电针对上睑下垂、眼球内收障碍、眼球下视受限疗效最好,对眼球上视运动受限疗效次之,对瞳孔症状疗效最差。在Logistic回归分析中治疗次数对痊愈的影响具有显著性(P<0.05),且OR值>1,即治疗次数会对痊愈产生显著的正向影响关系。结论眶内电针对动眼神经麻痹具有良好的疗效,对提上睑肌与内直肌、下直肌功能的恢复效果最好,对瞳孔功能的恢复效果较差,缺血性病因及炎症感染性病因患者的恢复情况较好。眶内电针治疗动眼神经麻痹治疗次数越多,痊愈的几率越大。患者性别、年龄、患眼、是否嗜烟嗜酒、发病天数、治疗前眼位评分、病因等并不会对动眼神经麻痹的痊愈产生影响。Objective To observe the efficacy of intraorbital electroacupuncture treatment for oculomotor nerve paralysis and the recovery pattern of patients during the treatment process.Methods A retrospective analysis was conducted on the clinical data of patients with oculomotor nerve paralysis who received intraorbital electroacupuncture treatment at the Eye Movement Disorder Treatment Center of Harbin Medical University from January 2020 to June 2023.The oculomotor nerve symptoms of the patients were scored,and the therapeutic effect was analyzed to observe the recovery pattern of oculomotor nerve paralysis patients during the treatment process.And it analyzed the relevant factors that affect recovery.Results A total of 221 patients with oculomotor nerve paralysis were included,of which 51 were cured,with a cure rate of 23.077%.There were 186 patients who were effective,significantly effective,and cured,with an overall effective rate of 84.163%.The scores of various aspects of oculomotor nerve paralysis in patients were significantly reduced,and the differences were statistically significant(P<0.05).Among them,orbital electroacupuncture had the best therapeutic effect on upper eyelid ptosis,intraocular adduction disorders,and inferior vision limitation,followed by superior vision limitation,and had the worst therapeutic effect on pupil symptoms.In Logistic regression analysis,the impact of treatment frequency on recovery was significant(P<0.05),and the OR value was greater than 1,indicating a significant positive relationship between treatment frequency and recovery.Conclusion Intraorbital electroacupuncture has a good therapeutic effect on oculomotor nerve paralysis,with the best recovery effect on the function of the levator palpebrae muscle,rectus muscle,and rectus muscle.The recovery effect on pupil function is poor,and patients with ischemic and inflammatory infectious causes have better recovery.The more treatments,the greater the chance of recovery from oculomotor nerve paralysis treated with intraorbital electr
