以“温通”为法探讨热疗联合贴敷治疗肺癌术后慢性疼痛可行性

摘　　要：术后慢性疼痛(chronic post-surgical pain,CPSP)是肺癌术后常见的并发症。中医认为,CPSP的发病与机体阳虚寒凝有关。阳气不足导致气血运行不畅,进而产生气滞、痰凝、血瘀等病理产物聚集,形成肿瘤。肿瘤因自身生长,耗伤机体气血,进一步加剧气血津液的失调,最终导致“不通则痛”或“不荣则痛”,从而引发CPSP。此外,局部气虚血瘀、痹阻经络也是CPSP难以治愈的关键因素。因此,在治疗CPSP时,应以“温经通络”为法,可以联合使用深部热疗和穴位贴敷缓解疼痛。深部热疗是一种现代治疗方法,通过提高局部温度来促进血液循环,从而缓解疼痛。中医认为,深部热疗的作用偏于温性,类似于“少火生气”的效果,能够激发体内阳气,发挥其温煦作用。中药贴敷疗法则是将药物研磨成细末,制成糊状,直接贴敷于痛处或穴位,药物成分经皮肤渗透至腠理,发挥通经活络的功效。根据王沛教授提出的治疗癌性疼痛的三大法则——“温通、行气、活血化瘀”,在选药时以温通为主,采用自拟的“丁香止痛方”发挥温通止痛之功。将深部热疗和中药贴敷疗法联合使用,能够更有效地缓解CPSP的症状,发挥协同治疗作用,为临床治疗CPSP提供了新的思路和方法。Chronic post-surgical pain(CPSP)is a common complication after lung cancer surgery.Traditional Chinese medicine believes that the onset of CPSP is related to the deficiency of Yang and the congealing of cold within the body.Insufficient Yang energy leads to poor circulation of Qi and blood,which in turn leads to the accumulation of pathological products such as Qi stagnation,phlegm coagulation,and blood stasis,forming tumors.The tumor,in its growth,consumes the body's Qi and blood,further exacerbating the imbalance of Qi,blood,and body fluids,ultimately leading to“pain from obstruction”or“pain from lack of nourishment,”thus causing CPSP.In addition,local Qi deficiency and blood stasis,as well as the blockage of meridians,are also key factors that make CPSP difficult to cure.Therefore,in treating CPSP,the method should focus on,and deep heat therapy and acupoint plastering can be used in combination to alleviate pain.Deep heat therapy is a modern treatment method that promotes blood circulation and relieves pain by increasing local temperature.Traditional Chinese medicine considers the effect of deep heat therapy to be warm,similar to the effect of“gentle fire promoting life”,which can stimulate the body's Yang energy and exert its warming action.Chinese herbal plaster therapy involves grinding the medicine into a fine powder,making it into a paste,and directly applying it to the painful area or acupoints,allowing the medicinal components to penetrate the skin into the interstitial spaces,playing the role of unblocking the meridians and activating the collaterals.According to Professor WANG Pei's three major principles for treating cancer pain--“warming and unblocking,promoting Qi circulation,and activating blood circulation to remove blood stasis”--the selection of medicine mainly focuses on warming and unblocking,using the self-devised“Dingxiang Zhitong Decoction(丁香止痛方)”to exert the effect of warming and unblocking for pain relief.The combined use of deep heat therapy and Chinese he
