基于血管重塑理论探讨脉络学说对早期高血压的防治

摘  要:早期高血压(early hypertension,EH)是介于正常血压与确诊高血压病的过渡阶段,此阶段虽未达诊断标准,但已出现血管内皮损伤及亚临床靶器官损害,因此,早期干预对于延缓高血压进程至关重要。通过分析EH的主要病理变化得知内皮功能障碍是血管重塑的起始环节,而常见不良生活习惯如吸烟、睡眠不足等会对内皮功能产生损害,从而通过氧化应激、炎症反应等途径最终导致血管重塑和血压升高。脉络学说作为中医理论的重要组成部分,其“脉络-血管系统病”概念与血管重塑密切相关。该文阐述了脉络学说中保护微血管内皮细胞的理念,与现代医学关于血管重塑的防治策略相呼应。现有研究成果提示通络药物能够通过多种机制调节内皮功能,恢复血管稳态、抑制血管重塑,有效防治EH。基于血管重塑理论和脉络学说,中医药在防治早期高血压方面具有独特优势,为延缓高血压进程及减少靶器官损害提供了新的研究思路和治疗手段。未来研究应进一步探讨通络药物等相关药物的具体作用机制,以推动其在临床实践中的广泛应用。Early hypertension(EH)represents a transitional stage between normal blood pressure and diagnosed hypertension.Although it does not meet the diagnostic criteria,early hypertension is characterized by endothelial injury and subclinical target organ damage.Consequently,early intervention is crucial for delaying the progression of hypertension.Analysis of the primary pathological changes in early hypertension reveals that endothelial dysfunction is the initiating step in vascular remodeling.Common adverse lifestyle habits such as smoking and insufficient sleep impair endothelial function,ultimately leading to vascular remodeling and elevated blood pressure through mechanisms including oxidative stress and inflammation.As a vital component of traditional Chinese medicine theory,collateral theory,with its concept of“vessel-collateral-vascular system disease,”is closely related to vascular remodeling.This paper elaborates on the concept within collateral theory of protecting microvascular endothelial cells,which echoes modern medical strategies for preventing and treating vascular remodeling.Existing research findings suggest that collateral-unblocking medications can regulate endothelial function through various mechanisms,restore vascular homeostasis,inhibit vascular remodeling,and effectively prevent and treat early hypertension.Drawing on theories of vascular remodeling and collateral circulation,traditional Chinese medicine presents unique advantages in preventing and treating early hypertension,offering novel research avenues and therapeutic strategies for delaying hypertension progression and mitigating target organ damage.Future research should delve deeper into the specific mechanisms of action of collateral-unblocking medications and related therapies to facilitate their wider application in clinical practice.
