摘　　要：血管性痴呆是一种由脑血管疾病导致的痴呆综合征,以学习、记忆等认知功能下降为主要表现,降低了患者的生活自理能力,给社会和家庭带来沉重的负担。目前该病发病机制仍不明确,西医治疗以胆碱酯酶抑制剂和N-甲基-D-天门冬氨酸(NMDA)拮抗剂为主,疗效甚微且不良反应大。近年来,中药治疗血管性痴呆的临床和动物研究逐渐增多。研究表明中医方剂及中药有效成分能通过调控氧化应激、神经炎症、神经胆碱能系统、病理蛋白的产生,抑制细胞凋亡,调控肠道菌群,调节线粒体功能和能量等机制,达到治疗血管性痴呆的作用。该文基于近20年的国内外文献,归纳总结了中药治疗血管性痴呆的研究进展,以期为血管性痴呆的中医药深入研究提供参考。Vascular dementia is a kind of dementia syndrome caused by cerebrovascular disease,which is mainly manifested by cognitive function decline such as learning and memory,and reduces the patient's ability to take care of themselves and brings a heavy burden to the society and family.At present,the pathogenesis of this disease is still unclear,and western medical treatment is based on cholinesterase inhibitors and N-methyl-D-aspartate(NMDA)antagonists,which have little efficacy and big side effects.In recent years,clinical and animal studies on the treatment of vascular dementia with traditional Chinese medicine(TCM)have gradually increased.Studies have shown that TCM formulas and active ingredients of TCM can achieve the therapeutic effect of vascular dementia(VD)by regulating oxidative stress,neuroinflammation,neurocholinergic system,production of pathological proteins,inhibition of apoptosis,modulation of intestinal flora,and regulation of mitochondrial function and energy,and other mechanisms.Based on nearly 20 years of reading domestic and international literature,this paper summarizes the research progress of traditional Chinese medicine in the treatment of vascular dementia,with a view to providing reference for the in-depth study of traditional Chinese medicine in vascular dementia.
