基于脑肠菌轴探讨合募配穴治疗功能性消化不良作用机制

摘　　要：功能性消化不良(functional dyspepsia,FD)是一种常见的功能性的,无胃肠道结构、形态及生化异常的胃肠道疾病,其病情绵延,变化复杂,长期不良反应明显。现代研究认为其发病机制与胃肠动力异常、内脏高敏感性、胃肠黏膜炎症反应、局部免疫失衡、精神心理障碍、肠道菌群失衡、慢性应激等脑-肠-菌轴(BGMA)失调关系密切。合募配穴法作为中医学诊治疾病的特色配穴方法,其目的是通过上下远近配穴,一升一降,纵横交织以求消除腑病。本病病位在胃,中医学认为,一方面“合治内府”即胃下合穴主治胃肠病证,二是“阳病行阴”“从阴引阳”则用属阴之胃募穴来治疗属阳之胃肠病,这为合募二穴治疗胃肠疾病奠定了丰富的理论基础。该文全面阐述了胃募穴及胃下合穴治疗胃肠疾病的理论依据及现代临床应用,并基于脑-肠-菌轴功能失调与功能性消化不良的生理病理关系,探析合募配穴治疗功能性消化不良的内在作用机制,以期为针灸合募配穴治疗功能性消化不良提供更好的治疗思路和研究方向。Functional dyspepsia(FD)is a common functional gastrointestinal disease without structure,morphology and biochemical abnormalities of the gastrointestinal tract.Modern studies believe that the pathogenesis is closely related to the brain-gut-microbiota axis(BGMA)disorders,such as abnormal gastrointestinal motility,visceral hypersensitivity,gastrointestinal mucosal inflammatory response,local immune imbalance,mental and psychological disorders,intestinal flora imbalance,and chronic stress.As a characteristic acupoint method for the diagnosis and treatment of diseases in traditional Chinese medicine,the purpose of combination of he-sea and front-mu points is to eliminate internal diseases by matching acupoints up and down,one up and one down,vertical and horizontal.Traditional Chinese medicine believes that on the one hand,“lower-he sea point for treating internal visceral diseases”,that is,the lower gastric acupoints are used to treat gastrointestinal diseases,and the other is that“Yang disease leads to Yin”and“introducing Yang from Yin”to treat the gastrointestinal tract of Yang,which has laid a rich theoretical foundation for the treatment of gastrointestinal diseases by Hezhao Erxue.Therefore,this article comprehensively expounds the theoretical basis and modern clinical application of gastric acupoints and hypogastric acupoints in the treatment of gastrointestinal diseases,and analyzes the internal mechanism of combination of he-sea and front-mu points in the treatment of functional dyspepsia based on the physiological and pathological relationship between BGMA dysfunction and functional dyspepsia,in order to provide better treatment ideas and research directions for combination of he-sea and front-mu points in the treatment of functional dyspepsia.
