摘  要:顽固性高血压(resistant hypertension,RH)患者比高血压病患者更易患慢性肾脏病、糖尿病、心血管疾病(cardiovascular diseases,CVD),且发生病死的风险更高。RH患者经常需要服用多种抗高血压药物来帮助缓解症状,但他们的血压仍然控制不佳,同时给社会带来了巨大的经济负担。RH患者的血压升高归因于肾素-血管紧张素-醛固酮系统(renin-angiotensin-aldosterone system,RAAS)激活增加、交感神经系统过度活跃和内皮素-1(endothelin-1,ET-1)水平升高。此外,肥胖和阻塞性睡眠呼吸暂停综合征(obstructive sleep apnea syndrome,OSAS)与顽固性高血压密切相关,影响其严重程度。该文阐述了有氧运动通过调节交感神经和迷走神经之间的动态平衡、调节RAAS以减少体液潴留、降低ET-1水平以及减轻肥胖和OSAS的程度来有效降低RH患者的血压。Patients with intractable hypertension(RH)are more likely to suffer from chronic kidney disease,diabetes,cardiovascular disease(CVD),and have a higher risk of death than patients with hypertension.Even though the patients with RH are often need to take a combination of antihypertensive medications in help to relieve their symptoms,their blood pressure is still poorly controlled,and at the same time,imposing a significant financial burden on society.Blood pressure elevation in RH patients is attributed to increased activation of the renin-angiotensin-aldosterone system(RAAS),higher sympathetic nervous system activity,and elevated levels of endothelin-1(ET-1).Additionally,obesity and obstructive sleep apnea syndrome(OSAS),which are seem to be the closely relative factors to resistant hypertension,may also affect the severity of RH.This article explained that aerobic exercise can effectively reduce the blood pressure of patients with RH by regulating the dynamic balance between sympathetic nerve and vagus nerve,regulating RAAS to reduce body fluid retention,reducing the level of ET-1 and the degree of obesity and OSAS.
