基于足细胞损伤探讨中医药干预治疗IgA肾病研究进展

摘　　要：免疫球蛋白A(IgA)肾病作为临床常见的肾小球疾病,其病因与发病机制复杂且多变,近年来一直是基础与临床研究的重要课题。其中,足细胞损伤作为IgA肾病病理过程中的新研究热点,备受关注。该文从中医络病、癥瘕、玄府以及干血等理论视角进行总结,概述中医药在IgA肾病治疗过程中针对足细胞损伤修复方面通过自拟方、中药提取物、中成药等治疗手段减缓肌动蛋白系统异常、足突融合、足细胞骨架蛋白的损伤修复,从而达到减少蛋白尿、保护肾脏功能的潜力和优势。基于上述理论不仅对IgA肾病的病理机制认识提供了新思路,也为中医药的干预治疗提供了有力的理论支持。凸显中医药从现代热点研究切入干预IgA肾病及中医治疗方面的特色和优势,为IgA肾病的中医药治疗探索新路径。IgA nephropathy is a common glomerular disease in clinical practice,which has been a major research topic in both basic and clinical studies due to its complex and variable etiology and pathogenesis.Among them,podocyte injury has become a new research hotspot in the pathological process of IgA nephropathy,which has attracted much attention.This article summarizes from the perspectives of traditional Chinese medicine(TCM)lymphatic disease,fibroids,pore and dry blood,and outlines the potential and advantages of TCM in treating IgA nephropathy by targeting podocyte injury repair through self-formulated prescriptions,herbal extracts,and patent medicines.Based on these theories,not only do they provide new insights into the pathogenesis of IgA nephropathy,but also provide powerful theoretical support for TCM intervention.This highlights the unique features and advantages of TCM in treating IgA nephropathy by integrating into modern hotspot research,and exploring new paths for TCM treatment of IgA nephropathy.
